眼睑动静脉畸形伴动静脉瘘1例

摘　　要：患者女性,40岁。因右眼眼睑皮下包块遮挡视野就诊。眼部查体:右眼上睑偏内侧可见一皮下肿块,局部皮肤呈紫红色,质软,无压痛,可触及与心律同步的搏动伴吹风样杂音,皮温增高,压迫眼睑遮挡眼球。入院后完善检查,诊断为右眼睑动静脉畸形伴动静脉瘘,于心血管外科行引流动静脉栓塞术,术后2个月包块未见明显减小,眼科行手术切除术,术后6个月右眼睑包块未复发。A 40-year-old female patient presented with complaint of visual field obstruction due to subcutaneous mass beneath the right eyelid.Eye examination showed a subcutaneous mass on the medial side of the upper eyelid of the right eye.The local skin was purplish red,soft,and nontender to pressure.This mass was pulsating with the heartbeat accompanied by a blowing-like murmur.It pressed against the eyelid,obstructing the eyeball and resulting in an increased skin temperature.Finally,the patient was diagnosed with right eyelid arteriovenous malformation with arteriovenous fistula through complete examination.The mass had not shown significant reduction at 2 months after interventional vascular embolization operation in the department of cardiovascular sugery,thus,surgical resection was then performed in the department of ophthalmology.The right eyelid mass had not recurred during a six-month follow-up after surgery.
