摘 要:目的观察针对老年宫颈癌同步放化疗患者以聚乙二醇化重组人粒细胞刺激因子(PEG-rhG-CSF)干预后对白细胞(WBC)、中性粒细胞(NEU)水平的影响。方法回顾性纳入2021年5月至2024年5月西安交通大学第一附属医院收治的老年宫颈癌患者78例作为研究对象。按照治疗方法不同将其分为研究组40例和对照组38例。所有患者均接受同步放化疗,研究组给予PEG-rhG-CSF治疗(6 mg/周期),对照组接受rhG-CSF治疗。观察治疗期间第1周期化疗前24 h(A_(0))、第1周期化疗后24~48 h(A_(1))、第1周期化疗后7 d(A_(2))及第2周期化疗前24 h(B_(0))、第2周期化疗后24~48 h(B_(1))、第2周期化疗后7 d(B_(2))的WBC计数、NEU计数、辅助T细胞(Th)1/Th2比例变化及NEU减少症和药物不良反应发生情况。结果两组WBC计数在化疗后时间、组间上差异均有统计学意义(P<0.05);研究组在A_(2)、B_(2)时间点的WBC计数分别为(8.08±2.54)×10^(9)/L、(7.42±1.67)×10^(9)/L,均明显高于对照组[(7.12±2.16)×10^(9)/L、(6.52±1.50)×10^(9)/L],差异均有统计学意义(P<0.05)。两组NEU计数在化疗后时间、组间、交互上差异均有统计学意义(P<0.05);研究组在A_(2)、B_(2)时间点的NEU计数分别为(7.58±2.14)×10^(9)/L、(5.98±1.52)×10^(9)/L,均明显高于对照组[(6.15±1.76)×10^(9)/L、(5.12±1.36)×10^(9)/L],差异均有统计学意义(P<0.05)。研究组NEU减少症的分级程度明显优于对照组,差异有统计学意义(P<0.05)。两组Th1/Th2比例在化疗后不同时间、组间上差异均有统计学意义(P<0.05);研究组在A_(2)、B_(2)时间点的Th1/Th2比例分别为2.12±0.59、2.19±0.55,均明显高于对照组的(1.80±0.52、1.86±0.47),差异均有统计学意义(P<0.05)。两组药物总不良反应发生率比较,差异无统计学意义(P>0.05)。结论相比于rhG-CSF,老年宫颈癌同步放化疗患者给予PEG-rhG-CSF干预可在一定程度上减少WBC、NEU降低风险,且有助于促进Th1/Th2免疫平衡。Objective To observe the influence of polyethylene glycol recombinant human granulocyte colony-stimulating factor(PEG-rhG-CSF)on the levels of white blood cell(WBC)and neutrophils(NEU)in elderly patients with cervical cancer receiving concurrent chemoradiotherapy.Methods Seventy-eight elderly patients with cervical cancer in the First Affiliated Hospital of Xi'an Jiaotong University were included from May 2021 to May 2024,and they received concurrent chemoradiotherapy.Forty patients in the study group were treated with PEG-rhG-CSF(6 mg/cycle),and the remaining 38 patients in the control group carried out rhG-CSF treatment.The changes of WBC,NEU and helper T cell(Th)1/Th2 ratio,NEU reduction at 24 h before the first cycle of chemotherapy(A_(0)),24-48 h after the first cycle of chemotherapy(A_(1)),7 d after the first cycle of chemotherapy(A_(2)),24 h before the second cycle of chemotherapy(B_(0)),24-48 h after the second cycle of chemotherapy(B_(1)),7 d after the second cycle of chemotherapy(B_(2))and occurrence of adverse drug reactions were observed during treatment.Results There were significant differences in WBC count between the two groups after chemotherapy(P<0.05).The WBC counts of the study group at A_(2)and B_(2)time points were(8.08±2.54)×10^(9)/L and(7.42±1.67)×10^(9)/L,respectively,which were significantly higher than those of the control group[(7.12±2.16)×10^(9)/L,(6.52±1.50)×10^(9)/L],the differences were statistically significant(P<0.05).There were significant differences in NEU count between the two groups in time,group and interaction after chemotherapy(P<0.05).The NEU levels at A_(2)and B_(2)time points were(7.58±2.14)×10^(9)/L and(5.98±1.52)×10^(9)/L in the study group,which were significantly higher than those of the control group[(6.15±1.76)×10^(9)/L and(5.12±1.36)×10^(9)/L],the differences were statistically significant(P<0.05).The grading degree of NEU reduction in the study group was significantly milder than that in the control group,the difference was statistically signifi
