D-D、吸烟和Padua量表对肺癌并发静脉血栓栓塞症的预测价值

摘　　要：目的:构建肺癌患者住院期间并发静脉血栓栓塞症(VTE)的预测模型。方法:回顾性分析在医院住院治疗的250例肺癌患者,根据住院期间是否并发VTE将患者分为肺癌(LC)组(肺癌未并发VTE,150例)和LC-VTE组(肺癌并发VTE,100例)。收集纳入患者当次入院信息及检验指标,采用Logistic回归分析肺癌患者并发VTE的相关因素,采用受试者工作特征(ROC)曲线评价相关指标对LC-VTE的预测价值。结果:LC组患者吸烟史比例、Padua量表得分、腺癌比例、白细胞计数、中性粒细胞计数、NLR、PLR、D-二聚体(D-D)、纤维蛋白原、谷草转氨酶均低于LC-VTE组(P<0.05),其BMI、淋巴细胞计数、LMR均高于LC-VTE组(P<0.05)。多因素Logistic回归分析结果显示,吸烟史、Padua量表及D-二聚体是肺癌患者发生VTE的独立影响因素。ROC曲线分析显示,吸烟史及Padua量表联合D-二聚体时,预测LC-VTE的曲线下面积为0.859,敏感度为74.0%,特异度为86.7%。结论:D-二聚体、吸烟史及Padua量表联合对LC-VTE有一定预测价值。Objective:To construct a prediction model of venous thromboembolism(VTE)in patients with lung cancer during hospitalization.Methods:A retrospective analysis was made on 250 patients with lung cancer in hospital.The patients were divided into lung cancer(LC)group(lung cancer without VTE,150 cases)and LC-VTE group(lung cancer complicated with VTE,100 cases)according to whether they were complicated with VTE during hospitalization.The admission information and test indicators were collected,and the factors related to VTE in lung cancer patients were analyzed by Logistic regression.The predictive value of relevant indicators on LC-VTE was evaluated by receiver operating characteristic(ROC)curve.Results:Smoking history ratio,Padua scale score,adenocarcinoma ratio,white blood cell count,neutrophil count,NLR,PLR,D-dimer,fibrinogen and glutamic oxalic transaminase in LC group were all lower than those in LC-VTE group.BMI,lymphocyte count and LMR were higher than those in LC-VTE group.Multivariate Logistic regression analysis showed that smoking history,Padua scale and D-dimer were independent influencing factors of VTE in lung cancer patients.ROC curve analysis showed that when smoking history and Padua scale combined with D-dimer,the predicted area under the curve of LC-VTE was 0.859,the sensitivity was 74.0%,and the specificity was 86.7%.Conclusion:The combination of D-dimer,smoking history and Padua scale has certain predictive value for LC-VTE.
