摘　　要：目的探讨帕金森病(PD)、原发性震颤(ET)和神经元核内包涵体病(NIID)的震颤特点。方法采集73例震颤患者(包括PD组30例、ET组23例、NIID组20例)的双上肢表面肌电图,采用功率谱分析探讨频率特点。通过单因素方差分析和卡方检验比较三组患者表面肌电图的各项电生理参数。结果ET的震颤频率高于PD(F=41.86,P<0.01),PD的震颤频率高于NIID静止状态(F=41.86,P=0.002)和NIID姿势状态(F=41.86,P=0.011)。PD交替收缩模式所占的比例高于NIID静止状态(χ^(2)=5.70,P=0.017)和姿势状态(χ^(2)=7.24,P=0.007),同时高于ET(χ^(2)=9.67,P=0.002)。PD谐波共振所占的比例同样高于NIID静止状态(χ^(2)=4.64,P=0.031)和姿势状态(χ^(2)=7.73,P=0.005)以及ET(χ^(2)=6.52,P=0.011)。结论三组比较ET的震颤频率最高,NIID的震颤频率最低,PD交替收缩模式和谐波共振所占的比例高于ET和NIID。Objective To investigate the characteristics of tremor in Parkinson disease(PD),essential tremor(ET),and neuronal intranuclear inclusion disease(NIID).Methods The surface electromyography(sEMG)data of both upper limbs were collected from 73 patients with tremor(30 patients in PD group,23 in ET group,and 20 in NIID group),and the a power spectral analysis was used to investigate frequency characteristics.A one-way analysis of variance and the chi-square test were used for comparison of electrophysiological parameters on sEMG between the three groups.Results The ET group had a higher tremor frequency than the PD group(F=41.86,P<0.01),while the PD group had a higher tremor frequency than the NIID group in resting state(F=41.86,P=0.002)and in postural state(F=41.86,P=0.011).The PD group had a higher proportion of patients with alternating contractions than the NIID group in resting state(χ^(2)=5.70,P=0.017)and in postural state(χ^(2)=7.24,P=0.007),as well as a higher proportion of such patients than the ET group(χ^(2)=9.67,P=0.002).The PD group also had a higher proportion of patients with harmonic resonances than the NIID group in resting state(χ^(2)=4.64,P=0.031)and in postural state(χ^(2)=7.73,P=0.005),as well as a higher proportion of such patients than the ET group(χ^(2)=6.52,P=0.011).Conclusion The highest tremor frequency is observed in ET,while the lowest tremor frequency is observed in NIID;patients with PD have a higher proportion of individuals with alternating contractions or harmonic resonances than patients with NIID and ET.
