机械取栓在青少年急性缺血性大血管闭塞中的临床疗效

摘　　要：目的 探讨机械取栓在青少年急性缺血性大血管闭塞(TAIS-LVO)中的临床疗效。方法 收集2017年1月至2021年12月于联勤保障部队第900医院行机械取栓治疗的5例TAIS-LVO患者的临床资料,记录患者的手术时间、取栓次数、美国国立卫生研究院卒中量表(NIHSS)评分及相关治疗指标。结果 5例患者手术成功率为80%,平均取栓次数1.8次,中位手术时间45 min,术后即刻、24 h、出院时中位NIHSS评分依次为10、10、3分。1例脂肪栓塞患者术后48 h内发生症状性颅内出血、脑水肿继发脑疝,最终因多系统功能衰竭抢救无效死亡,其他4例患者均未出现明显并发症。术后90 d改良Rankin量表(m RS)评分良好预后率为80%。结论 机械取栓治疗TAIS-LVO具有良好的可行性及安全性。Objective To investigate the clinical effect of mechanical thrombectomy in adolescents with acute ischemic large vessel occlusion(TAIS-LVO).Method The clinical data of 5 patients with TAIS-LVO who underwent mechanical thrombectomy at the 900th Hospital of the Joint Logistics Support Force from January 2017 to December 2021 were collected,and the operative time,thrombectomy times,National Institutes of Health Stroke Scale(NIHSS)score and related treatment indexes were recorded.Result The operative success rate of the 5 patients was 80%,the average number of thrombectomy was 1.8 times,the median operative time was 45 min,and the median NIHSS score was 10,10,and 3 points immediately after surgery,24 h,and at discharge,respectively.One patient with fat embolism developed symptomatic intracranial hemorrhage and cerebral edema secondary cerebral hernia within 48 h after surgery,and eventually died due to multi-system failure.The other 4 patients had no obvious complications.The good prognosis rate of the modified Rankin scale(mRS)at 90 days after surgery was 80%.Conclusion Mechanical thrombectomy has good feasibility and safety in the treatment of TAIS-LVO.
