摘　　要：扩张型心肌病是一种病死率高、预后差的心肌病。患者早期症状不明显,随着病情的发展,其心脏扩大,导致心力衰竭,出现呼吸困难、水肿甚至咯血等一系列症状,并发心律失常、胸腔积液等多种疾病,严重者会发生心源性猝死,已成为心血管系统疾病的一大治疗难题,临床上中医、西医单独使用后治疗效果一般,用药的局限性大、预后差,中西医结合治疗在改善患者症状上有明显的优势,能更好地提高患者的幸福指数。张世亮教授是山东省第二批名老中医林慧娟主任医师的学术继承人,是第三、第五批全国老中医药专家学术经验继承工作指导老师,在扩张型心肌病的病因病机上,认为痰饮瘀血、饮食失常、劳逸失当、情志内伤等是其基本病因,脾胃阳虚、瘀阻心脉是基本病机,中西医并重,将中药和西药灵活结合使用,在临床上以温中散寒、活血化瘀为治疗原则辨证施治,能有效改善心脏的结构与功能,治疗效果佳。本文通过1则案例介绍张世亮教授治疗扩张型心肌病的临床经验,以期为临床实践和学术研究提供参考。Dilated cardiomyopathy is a kind of cardiomyopathy with high mortality and poor prognosis.The early symptoms of patients are not obvious.With the development of the disease,the patients’heart dilates,leading to heart failure,a series of symptoms such as dyspnea,edema and even hemoptysis,complicated by arrhythmia,pleural effusion and other diseases,and sudden cardiac death in severe cases.It has become a major problem in the treatment of cardiovascular system diseases,the effect of clinical treatment of traditional Chinese medicine and Western medicine alone is general,there are limitations of the drug,the prognosis is poor.The combination of traditional Chinese medicine and Western medicine has obvious advantages in relieving the symptoms of patients,and can better improve the happiness index of patients.Professor ZHANG Shiliang is the academic successor of Chief Physician LIN Huijuan,one of the Second Batch of Famous TCM practitioners in Shandong Province,and the instructor of the Academic Experience Inheritance Work of the Third and Fifth Batch of Veteran TCM Eexperts in China.In terms of etiology and pathogenesis of dilated cardiomyopathy,Professor ZHANG Shiliang believes that phlegm and fluid stasis,diet disorder,lack of appropriate exercise,emotional internal injury,etc.,are the basic etiology of the disease,and spleen and stomach Yang(阳)deficiency and blood stasis of the heart and meridian are the basic pathogenesis of the disease.Professor ZHANG attaches equal importance to traditional Chinese medicine and Western medicine,flexibly combines traditional Chinese medicine and Western medicine.In clinical practice,the treatment principle of warming the middle to disperse cold and activating blood stasis is the principle of syndrome differentiation,which can effectively improve the structure and function of the heart,and the treatment effect is good.This article introduces Professor ZHANG Shiliang’s clinical experience in the treatment of dilated cardiomyopathy through one test case,in order to provide fu
