摘 要:目的:研究肺腺癌EGFR基因突变与X射线断层扫描(computed tomography,CT)表现的相关性,找到能有效预测EGFR基因突变的无创方法,指导临床工作。方法:纳入福建省福州肺科医院2020年1月1日~2023年5月31日病理证实为肺腺癌的患者247例,回顾性分析所有患者的影像资料及EGFR基因突变表型。所有CT征象均由2位高年资影像主治医师利用(picture archiving and communication systems,PACS)系统共同进行图像分析并记录结论,采用单因素及多因素分析数据得出结论。结果:EGFR突变与女性、远处转移相关;EGFR外显子19突变与血管聚集征阳性相关;EGFR外显子20突变与胸腔积液和年龄相关;EGFR外显子21突变与远处转移、年龄及毛刺征相关。结论:肺腺癌EGFR基因突变与高分辨CT征象具有一定相关性;HRCT在一定程度上可以预测EGFR基因突变。Objective:Investigate the correlation between EGFR gene mutation in pulmonary adenocarcinoma and CT findings,and seek a noninvasive method to predict EGFR gene mutation effectively and guide the clinical practices.Methods:247 patients with pathologically confirmed lung adenocarcinoma from Fuzhou Pulmonary Hospital in Fujian Province from January 1,2020 to May 31,2023 were included.The imaging data and EGFR gene mutation phenotype of all patients were retrospectively analyzed.All CT signs were analyzed and conclusions were recorded by two senior radiologists using a picture archiving and communication systems(PACS)system.Single factor and multi factor analysis data were used to draw conclusions.Results:EGFR mutations were associated with female and distant metastasis;EGFR exon 19 mutation was associated with positive vascular aggregation sign;EGFR exon 20 mutation was associated with pleural effusion and age;EGFR exon 21 mutations were associated with distant metastasis,age,and spiculation.Conclusion:EGFR gene mutation in pulmonary adenocarcinoma is correlated with CT Findings to some extent;HRCT may predict EGFR gene mutation to some extent.
