摘　　要：目的:探讨Solitaire FR型支架取栓治疗急性大动脉闭塞性脑梗死的临床疗效及安全性。方法:选取2021年1月~2023年12月在无锡市人民医院神经外科接受治疗的80例急性大动脉闭塞性脑梗死患者,根据入院制定的患者编号进行单双数随机分为对照组(n=40)和研究组(n=40),对照组给予常规药物治疗,研究组在常规治疗基础上采用Solitaire FR型支架取栓治疗。比较两组患者的再通率、症状性颅内出血发生率、90d良好预后率、神经功能缺损评分(NIHSS)及改善程度。结果:研究组再通率显著高于对照组(P<0.05);症状性颅内出血发生率低于对照组(P<0.05);90d良好预后率高于对照组(P<0.05)。研究组治疗后24h、7d、90d的NIHSS评分均显著低于对照组(P<0.05),NIHSS评分改善程度更明显(P<0.05)。结论:Solitaire FR型支架取栓治疗急性大动脉闭塞性脑梗死能显著提高血管再通率,改善患者神经功能,提高预后,且具有较好的安全性。Objective:To investigate the clinical efficacy and safety of Solitaire FR in acute artery occlusive cerebral infarction.Methods:For 80 cases of acute major artery occlusive cerebral infarction treated in Neurosurgery Department of Wuxi People’s Hospital from January 2021 to December 2023.The patients were randomly divided into a control group(n=40)and a study group(n=40)according to the patient numbers assigned on admission.The control group was given conventional drug treatment,and the study group was treated with Solitaire FR stent thrombectomy on the basis of conventional treatment.The recanalization rate,incidence of symptomatic intracranial hemorrhage,90d good outcome,neurological deficit score(NIHSS)and improvement were patients in the two groups were patients.Results:The recanalization rate was significantly higher than the control group(P<0.05);The incidence of symptomatic intracranial hemorrhage was lower than the control group(P<0.05);And the 90d good outcome rate was higher than the control group(P<0.05).The NIHSS score at 24h,7d and 90d in the study group were significantly lower than those of the control group(P<0.05),and the improvement of NIHSS score was more obvious(P<0.05).Conclusion:Solitaire FR stent graft thrombolysis for acute large artery occlusion cerebral infarction can significantly improve vascular recanalization rate,improve patient nerve function,improve prognosis,and have good safety.
