摘 要:目的:探讨不同CT管电流值对肺癌放疗靶区勾画范围的影响。方法:选取2020年11月~2023年10月本院收治的肺癌患者100例,根据患者胸部实际体积将患者分为A组(胸部实际体积≤1cm^(2))28例、B组(1cm^(2)<胸部实际体积≤6cm^(2))24例、C组(6cm^(2)<胸部实际体积≤12cm^(2))22例、D组(12cm^(2)<胸部实际体积≤18cm^(2))26例,对四组患者均进行电流为80mA、120mA、160mA、200mA、240mA、280mA的CT扫描,记录CT影像。将患者CT图像利用TPS放疗计划系统扫描患者胸部体积,记录肺部组织勾画的靶区体积。结果:不同电流对肺癌放疗靶区勾画体积不同,电流为240mA和280mA时,A组、B组、C组、D组患者胸部肺癌放疗靶区勾画体积与患者实际体积比较无显著差异(P>0.05)。电流为80mA、120mA、160mA、200mA时,A组、B组、C组、D组患者胸部肺癌放疗靶区勾画体积均小于患者实际体积,差异具有统计学意义(P<0.05)。结论:在对肺癌患者进行扫描时,建议CT管电流在240~280mA区间内,电压为130kV,厚层为3.0mm,对肺癌放疗靶区勾画体积较为稳定且更接近于实际体积。Objective:To investigate the effect of different CT tube current values on the delineation of lung cancer radiotherapy target.Methods:100 patients with lung cancer admitted to our hospital from November 2020 to October 2023 were selected.According to the actual chest volume,the patients were divided into group A(actual chest volume≤1cm^(2)),28 cases,group B(1cm^(2)<actual chest volume≤6cm^(2)),24 cases,group C(6cm^(2)<actual chest volume≤12cm^(2)),22 cases,group D(12cm^(2)<actual chest volume≤18cm^(2)),26 cases.CT scans with current of 80mA,120mA,160mA,200mA,240mA and 280mA were performed for all patients in the four groups,and CT images were recorded.The patient’s chest volume was scanned by CT images using TPS radiotherapy planning system,and the target area volume sketched by lung tissue was recorded.Results:The sketched volume of lung cancer radiotherapy target was different with different currents.When the currents were 240mA and 280mA,there was no significant difference between the sketched volume and the actual volume of lung cancer radiotherapy target in groups A,B,C and D(P>0.05).When the current was 80mA,120mA,160mA,200mA,the sketchbook volume of thoracic lung cancer radiotherapy target area in groups A,B,C,and D was smaller than the actual volume of patients,and the difference was statistically significant(P<0.05).Conclusion:When scanning lung cancer patients,it is recommended that the current of CT tube should be in the range of 240-280mA,the voltage should be 130kV,and the thickness should be 3.0mm.The sketchable volume of lung cancer radiotherapy target area is relatively stable and closer to the actual volume.
