浅析裴正学教授治疗胰腺癌的临床经验

摘　　要：胰腺癌为临床上较常见的一种恶性消化道肿瘤,恶性程度极高,预后较差,中医中药在胰腺癌的综合治疗中占有重要地位。甘肃省肿瘤医院首席专家裴正学教授,从事中西医结合肿瘤防治60余年,认为中医扶正固本与西医放、化疗相结合形成互补,是当代内科领域治疗恶性肿瘤的有效模式。裴老认为早期胰腺癌仍以手术切除为首选方案,但术后复发转移极为常见,因此术后仍应长期服用中药,以防止复发及转移;对于中晚期胰腺癌一般不选用手术治疗,给予化疗、免疫等治疗,配合中药减轻化疗的不良反应,提高患者生活质量,延长患者的生存期;对于一般状况差无法行化疗的患者,可采用“围点打援”“围魏救赵”的方法,给予最佳支持、对症止痛治疗的同时,给予中药治疗。根据肝郁气滞、湿热蕴结等病机,以疏肝理气、清热泻火、活血化瘀为原则,创立治疗胰腺癌的有效方剂胆胰合症方,该方以柴胡疏肝散为主加减而成,熔行气、柔肝、清热、祛湿、活血于一炉,随症灵活加减治疗。结合一则临床随诊案例,总结裴正学教授中西医结合治疗胰腺癌的临床经验。Pancreatic cancer is a common malignant digestive tract tumor with high malignant degree and poor prognosis.Traditional Chinese medicine plays an important role in the comprehensive treatment of pancreatic cancer.Professor PEI Zhengxue,chief expert of Gansu Provincial Cancer Hospital,who has been engaged in tumor prevention and treatment with integrated traditional Chinese medicine for more than 60 years,believes that the combination of traditional Chinese medicine and Western medicine is an effective model for the treatment of malignant tumors in the contemporary field of internal medicine.Master PEI believes that surgical resection is still the first choice for early pancreatic cancer,but postoperative recurrence and metastasis of pancreatic cancer is very common,so long-term use of traditional Chinese med icine should be taken after surgery to prevent recurrence and metastasis.For intermediate and advanced pancreatic cancer,surgery is generally not selected,chemotherapy,immunization and other treatments are given,combined with traditional Chinese medicine to reduce the side effects of chemotherapy,improve the quality of life of patients,and extend the survival time of patients.For patients with poor general condition who cannot receive chemotherapy,the method of“besiege the stronghold to annihilate the enemy relief force”and“relieve the State of ZHAO by besieging the State of WEI”can be used to give the best support and symptomatic pain relief treatment on the basis of giving traditional Chinese medicine treatment.According to the pathogenesis of liver-qi stagnation,damp-heat accumulation and so on,and taking the principle of soothing liver and regulating Qi(气),clearing heat and expelling fire,activating blood and removing blood stasis,an effective prescription for treating pancreatic cancer,Danyi Hezheng formula(胆胰合症方)is created,the prescription is mainly based on Chaihu Shugan powder(柴胡疏肝散)and it is modified,which combines regulating Qi,soothing liver,clearing heat,removing damp
