中医康复疗法干预颈椎病慢性颈痛的脑网络机制研究进展

摘　　要：颈椎病是临床常见的慢性退行性肌肉骨骼系统疾病,颈痛是其最常见且最突出的临床症状,当疼痛迁延难愈、时间超过3个月及以上则形成慢性颈痛(CNP)。中医康复疗法广泛运用于CNP治疗中并取得较好疗效,但既往研究多关注中医康复疗法对CNP躯体障碍改善的作用机制,对此疗法在中枢发挥的作用机制尚不明确。本综述依据神经影像学检查方法,回顾总结CNP的脑网络机制改变及中医康复疗法干预CNP的中枢病理改变特征。结果表明CNP能够改变脑结构、影响脑功能和脑代谢水平,中医康复疗法可调节特定脑区功能并改善脑区之间功能连接,减轻脑网络区域异常状态的有害影响,恢复脑区结构和改善脑血流动力学,使患者原本异常的活动恢复至正常状态。这些发现为未来深入探讨中医康复疗法在CNP中的作用机制提供了神经影像学上的重要参考。Cervical spondylosis is a prevalent chronic degenerative musculoskeletal disorder in clinical practice,with neck pain being its most common and prominent clinical symptom.When the pain persists for more than 3 months,it develops into chronic neck pain(CNP).Traditional Chinese medicine rehabilitation therapy is widely utilized in CNP treatment and has demonstrated favorable therapeutic effects.However,previous studies have primarily focused on the improvement of bodily dysfunction mechanisms by traditional Chinese medicine rehabilitation therapy for CNP,while the central action mechanism remains unclear.This review summarizes changes in brain network mechanisms and central pathological changes of CNP intervention through traditional Chinese medicine rehabilitation therapy based on neuroimaging examination methods.The results indicate that CNP can alter brain structure,influence brain function and metabolism.Traditional Chinese medicine rehabilitation therapy can modulate specific brain regions'functions,enhance functional connectivity between brain regions,alleviate adverse effects of abnormal brain network regions,restore brain region structures,and improve cerebral blood flow dynamics to normalize patients'abnormal activities.These findings offer crucial neuroimaging references for further exploration of the mechanism behind traditional Chinese medicine rehabilitation therapy in treating CNP.
