摘 要:目的:探究重复经颅磁刺激对脑缺血再灌注大鼠皮层小胶质细胞极化的影响。方法:将SD大鼠随机分为假手术组、模型组和干预组,每组8只,采用线栓阻断大脑中动脉法制备脑缺血再灌注损伤大鼠模型。对干预组大鼠建模成功后24 h进行rTMS干预,每天1次,连续7天,对假手术组和模型组给予伪rTMS刺激。7天后对所有大鼠进行神经功能评分后进行取材,HE染色观察大鼠皮层脑组织及神经细胞结构,ELISA法检测大鼠血清中IL-1β、TNF-α、IL-10、和TGF-β_(1)的含量,荧光定量PCR和Western blot分别检测大脑皮质中IL-1β、TNF-α、CD206 mRNA和蛋白表达。结果:术后第2天,假手术组未见神经功能损伤,与假手术组相比,模型组大鼠神经功能评分均明显升高(P<0.0001);与模型组相比,干预组神经功能评分升高,但统计学无差异(P>0.05)。术后第8天,假手术组未见神经功能损伤,与假手术组相比,模型组大鼠神经功能评分明显升高(P<0.0001);与模型组相比,干预组神经功能评分明显下降(P<0.001)。模型组大鼠神经元排列较为紊乱,部分神经细胞死亡,细胞核结构欠完整,难以辨认;干预组大鼠大脑皮质神经元细胞损伤程度及细胞减少数量均较模型组有所减轻。与假手术组相比,模型组血清中IL-1β、TNF-α含量显著升高(P<0.001,P<0.0001),IL-10和TGF-β_(1)含量显著降低(P<0.001),大脑皮质中IL-1β、TNF-αmRNA表达及蛋白水平均显著升高(P<0.0001,P<0.001),CD206 mRNA表达显著降低(P<0.0001),CD206蛋白水平明显降低(P<0.05)。与模型组比较,干预组血清IL-1β、TNF-α含量显著降低(P<0.05,P<0.001),TGF-β_(1)含量显著升高(P<0.01),IL-10含量有升高趋势,但无统计学差异(P>0.05),大脑皮质中IL-1β、TNF-αmRNA表达显著降低(P<0.001,P<0.05),IL-1β、TNF-α蛋白水平明显降低(P<0.01),CD206 mRNA表达显著升高(P<0.01),CD206蛋白表达升高,但差异无统计学意义(P>0.05)。结论:重复经颅磁刺激�Objective:To investigate the effect of repetitive transcranial magnetic stimulation on the polarization of cortical microglial in cerebral ischemia-reperfusion rats.Methods:SD rats were randomly divided into a sham operation group,a model group and an intervention group,with 8 rats in each group,and a rat model of cerebral ischemia/reperfusion injury was prepared by using middle cerebral artery lineal thrombus blockade.Rats in the intervention group received rTMS intervention 24 h after successful modeling,once a day for 7 days,and pseudo-rTMS stimulation was given to the sham-operation group and the model group.7 days later,all the rats were scored for neurological function and then sampled,and HE staining was used to observe the histopathological morphology of rat cerebral cortex,and the serum levels of IL-1β,TNF-α,IL-10,and TGF-β_(1) were detected by ELISA.In addition,fluorescence quantitative PCR and Western blot were used to detect IL-1β,TNF-α,CD206 mRNA and protein expression in the cerebral cortex,respectively.Results:On the 2nd postoperative day,no neurological impairment was seen in the sham-operated group,and the neurological function scores of the rats in the model group were all significantly higher compared with the sham-operated group(P<0.0001);compared with the model group,the neurological function scores of the intervention group were elevated but statistically indistinguishable(P>0.05).On the 8th postoperative day,no neurological impairment was observed in the sham-operated group,and the neurological function scores of the rats in the model group were significantly elevated compared with those in the sham-operated group(P<0.0001);the neurological function scores of the intervention group were significantly decreased compared with those of the model group(P<0.001).In the model group,the neurons were arranged in a disorganized way,some nerve cells died,and the nuclei of the cells were incomplete and difficult to recognize;in the intervention group,the degree of neuronal cell damage in the cere
