高危型HPV联合TCT检测在宫颈癌筛查中的应用价值分析

摘　　要：目的探究宫颈癌患者筛查中高危型人乳头瘤病毒(HPV)联合液基薄层细胞检测(TCT检测)的应用价值。方法选取800例宫颈癌筛查人员作为研究对象,均进行高危型HPV检测及TCT检测。以病理学检查结果为金标准,分析高危型HPV、TCT单独检测及联合检测的诊断结果;比较高危型HPV、TCT单独检测及联合检测的诊断效能。结果病理检查结果显示:阳性30例,阴性770例。高危型HPV检测结果显示:阳性39例,阴性761例;TCT检测结果显示:阳性40例,阴性760例;联合检测结果显示:阳性31例,阴性769例。高危型HPV检测的诊断符合率为96.38%、敏感度为66.67%、特异度为97.53%、阳性预测值为51.28%、阴性预测值为98.69%;TCT检测的诊断符合率为96.75%、敏感度为73.33%、特异度为97.66%、阳性预测值为55.00%、阴性预测值为98.95%;联合检测的诊断符合率为99.63%、敏感度为96.67%、特异度为99.74%、阳性预测值为93.55%、阴性预测值为99.87%。联合检测的诊断符合率、敏感度、特异度、阳性预测值、阴性预测值均高于高危型HPV、TCT单独检测(P<0.05)。结论应用高危型HPV联合TCT检测对宫颈癌进行筛查,有利于诊断符合率、敏感度以及阴性预测值的提升,值得推广。Objective To explore the application value of high-risk human papillomavirus(HPV)testing combined with thinprep cytologic test(TCT)in cervical cancer screening.Methods 800 subjects with cervical cancer screening were selected as the research subjects,and all received high-risk HPV testing and TCT.The pathological examination results were used as the gold standard to analyze the diagnostic results of individual and combined testing of high-risk HPV testing and TCT;the diagnostic efficacy of individual and combined testing of high-risk HPV testing and TCT was compared.Results The pathologic results showed 30 positive cases and 770 negative cases;high-risk HPV testing showed 39 positive cases and 761 negative cases;TCT results showed 40 positive cases and 760 negative cases;the combined test showed 31 positive cases and 769 negative cases.The diagnostic accordance rate of high-risk HPV testing was 96.38%,the sensitivity was 66.67%,the specificity was 97.53%,the positive predictive value was 51.28%,and the negative predictive value was 98.69%;the diagnostic accordance rate of TCT was 96.75%,the sensitivity was 73.33%,the specificity was 97.66%,the positive predictive value was 55.00%and the negative predictive value was 98.95%;the diagnostic accordance rate of the combined testing was 99.63%,the sensitivity was 96.67%,the specificity was 99.74%,the positive predictive value was 93.55%,and the negative predictive value was 99.87%.The diagnostic accordance rate,sensitivity,specificity,positive predictive value,and negative predictive value of the combined testing were higher than those of high-risk HPV testing and TCT alone(P<0.05).Conclusion The combination of high-risk HPV testing and TCT for cervical cancer screening is beneficial for improving diagnostic accordance rate,sensitivity,and negative predictive value,and is worth promoting.
