柴胡加龙骨牡蛎汤加减联合利培酮治疗首发精神分裂症的临床疗效研究

摘　　要：目的探究柴胡加龙骨牡蛎汤加减联合利培酮治疗首发精神分裂症的临床疗效。方法以80例首发精神分裂症患者作为研究对象,以随机数字表法分为对照组及观察组,各40例。对照组采取利培酮治疗,观察组在对照组基础上增加柴胡加龙骨牡蛎汤加减治疗。对比两组患者简明精神病评定量表(BPRS)评分,神经营养因子[脑源性神经营养因子(BDNF)、神经生长因子(NGF)、神经营养因子-3(NT-3)]水平。结果治疗后,两组患者BPRS评分均低于治疗前,且观察组患者BPRS评分(62.31±3.72)分低于对照组的(64.10±4.21)分(P<0.05)。治疗后,两组患者BDNF、NGF、NT-3均高于治疗前,且观察组BDNF(5101.19±36.24)pg/ml、NGF(41.09±6.33)pg/ml、NT-3(168.41±15.34)pg/ml均高于对照组的(4560.33±30.34)、(36.44±6.57)、(146.23±16.09)pg/ml(P<0.05)。结论首发精神分裂症患者采取柴胡加龙骨牡蛎汤加减+利培酮治疗,可有效改善患者的精神病症状严重程度,提高患者的神经营养因子水平,利于患者预后,值得临床大力推广。Objective To explore the clinical efficacy of modified Bupleurum plus Longgu Muli decoction combined with risperidone in the treatment of first-episode schizophrenia.Methods A total of 80 cases of first-episode schizophrenic patients as study subjects were allocated into a control group and an observation group,with 40 patients in each group.The control group was treated with risperidone,and the observation group was treated with modified Bupleurum plus Longgu Muli decoction on the basis of the control group.The score of Brief Psychiatric Rating Scale(BPRS)and the levels of neurotrophic factor[brain-derived neurotrophic factor(BDNF),nerve growth factor(NGF)and neurotrophin-3(NT-3)]were compared between the two groups.Results After treatment,BPRS score in both groups was lower than that before treatment,and the observation group had lower BPRS score of(62.31±3.72)points than(64.10±4.21)points in the control group(P<0.05).After treatment,BDNF,NGF and NT-3 in both groups were higher than that before treatment;the observation group had BDNF of(5101.19±36.24)pg/ml,NGF of(41.09±6.33)pg/ml and NT-3 of(168.41±15.34)pg/ml,which were higher than(4560.33±30.34),(36.44±6.57)and(146.23±16.09)pg/ml in the control group(P<0.05).Conclusion Modified Bupleurum plus Longgu Muli decoction combined with risperidone can effectively improve the psychiatric rating scale,improve the level of neurotrophic factors and is beneficial to the prognosis of patients with first-episode schizophrenia.It is worthy of clinical promotion.
