基于“通阳健脾利水”法治疗以黄斑囊样水肿的Usher综合征1例

摘　　要：Usher综合征(USH)是临床上最常见的视网膜色素变性(RP)合并感音神经性听力损失(SNHL)的常染色体隐性遗传性疾病。黄斑囊样水肿(CME)是RP的常见并发症。目前没有任何疗法可以阻止Usher综合征的发展及彻底根治该病,只能对症治疗并发症以协助患者进行正常感官功能,减缓退化过程。而针对USH继发的CME一线方案是注射抗血管内皮生长因子(VEGF)药物,疗效局限且因人而异。通过介绍一例首诊USH1F型患儿,多次抗VEGF治疗无效后,经名老中医郝小波教授采用中医药“通阳健脾利水”之法成功治疗患儿顽固性黄斑囊样水肿病变,发挥中医药在诊治复杂性CME方面的独特优势,为从根本上治疗USH及CME的实验药物研究及临床指导提供参考。Usher syndrome(USH)is the most common clinical Retinitis Pigmentosa(RP)complicated by Sensorineural Hearing Loss(SNHL)is an autosomal recessive genetic disorder.cystoid macular edema(CME)is a common complication of RP.There is currently no treatment that can stop the development of Usher syndrome and completely cure the disease,only symptomatic treatment of complications to assist patients with normal sensory function and slow down the degeneration process.The first-line protocol for USH secondary CME is to inject anti-vascular endothelial growth factor(VEGF)drugs,and the efficacy is limited and varies from person to person.This article introduces a case of USH1F type child who was diagnosed for the first time.After several anti-VEGF treatments failed,Professor Hao Xiaobo,a famous old Chinese medicine practitioner,successfully treated the intractable cystoid macular edema of the child with the method of“promoting Yang to strengthen spleen and promoting water”,giving full play to the unique advantages of Chinese medicine in diagnosing and treating complex CME.To provide reference for experimental drug research and clinical guidance for the fundamental treatment of USH and CME.
