摘 要:目的:探讨子宫内膜癌(EC)的磁共振成像(MRI)特征、Ki-67蛋白表达水平与分化程度的相关性,并分析MRI特征和Ki-67蛋白表达对EC患者肿瘤低分化程度的预测价值。方法:选取2020年6月—2023年6月苏州大学附属第一医院收治的72例EC患者的MRI影像数据及Ki-67标记指数作为研究对象,根据EC分化程度分类标准将患者分为高分化组(n=28)、中分化组(n=31)、低分化组(n=13)。比较三组临床特征、不同分化程度的MRI特征、不同分化程度的Ki-67蛋白表达水平,并进行MRI特征和Ki-67蛋白表达对EC患者肿瘤分化程度的预测价值分析。结果:低分化组内膜增厚、宫颈间质浸润、宫旁受累、腹膜转移和腹股沟淋巴结肿大例数高于中、高分化组,Ki-67蛋白标记指数高于中、高分化组,差异有统计学意义(P<0.05)。MRI特征、Ki-67蛋白表达水平均与EC分化程度存在相关性。MRI特征、Ki-67蛋白表达联合预测EC分化程度的灵敏度为84.62%,特异度95.00%。结论:通过分析EC不同分化程度的MRI特征和Ki-67蛋白表达水平,可以有效辅助临床在诊断子宫内膜癌分化程度时做出更准确的判断。Objective:To explore the correlation between magnetic resonance imaging(MRI)features,Ki-67 protein expression levels,and the degree of differentiation in endometrial cancer(EC),as well as to analyze the predictive value of MRI features and Ki-67 protein expression in determining the degree of tumor differentiation in patients with EC.Method:MRI image data and Ki-67 protein marker index of 72 patients with EC admitted to the First Affiliated Hospital of Soochow University from June 2020 to June 2023 were selected as the study objects.According to the classification criteria of EC differentiation,the patients were divided into three groups:highly differentiated(n=28)group,moderately differentiated group(n=31)and poorly differentiated(n=13)group.The clinical features,MRI features of different degrees of differentiation and Ki-67 protein expression level of different degrees of differentiation were compared among all groups.The predictive value of MRI features and Ki-67 protein expression on tumor differentiation in patients with EC was analyzed.Result:The cases of endometrium thickening,cervical interstitial infiltration,paracentral involvement,peritoneal metastasis and inguinal lymph node enlargement in differentiated group were higher than those in moderately differentiated and highly differentiated groups,and the Ki-67 protein marker index was significantly higher than that in moderately differentiated and highly differentiated groups,with statistical significance(P<0.05).MRI features and Ki-67 protein expression level were correlated with the differentiation degree of EC.The sensitivity and specificity of MRI features and Ki-67 protein expression were 84.62%and 95.00%respectively in predicting the differentiation degree of EC.Conclusion:By analyzing MRI features and Ki-67 protein expression levels of EC with different degrees of differentiation,it can effectively assist clinical diagnosis of EC differentiation to make a more accurate judgment.
