摘  要:目的:研究脑梗死偏瘫患者恢复期接受早期伸肌康复治疗对于上肢功能恢复的影响。方法:选取2021年1月—2024年3月南京悦群医院收治的109例脑梗死偏瘫患者作为研究对象,按照奇偶数法分为对照组和研究组。对照组54例采用屈肌康复治疗,研究组55例采用早期伸肌康复治疗。比较上肢运动能力恢复情况、超声肌肉结构参数、上肢肌电信号、日常生活自理能力、生活质量。结果:治疗后,两组肢体运动功能评定量表(FMA)比较,研究组评分高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组肱二头肌的肌纤维长度、肌纤维厚度均高于对照组,回声强度低于对照组,差异有统计学意义(P<0.05)。治疗后,研究组肱二头肌、肱三头肌的均方根值以及伸肘、屈肘时协同收缩率高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组生活自理能力(BI)评分高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组生活质量自评量表(SF-36)各维度评分均高于对照组,差异有统计学意义(P<0.05)。结论:早期伸肌康复治疗可以促进脑梗死恢复期患者偏瘫上肢运动能力恢复,改善超声肌肉结构、肌电信号,提高日常生活自理能力、生活质量。Objective:To study the effect of early extensor rehabilitation on the recovery of upper limb function in patients with hemiplegia after cerebral infarction.Method:A total of 109 patients with cerebral infarction hemiplegia admitted to Yuequn Hospital of Nanjing from January 2021 to March 2024 were selected as the study objects and divided into control group and study group according to odd-even method.The control group(54 cases)received flexor rehabilitation,and the study group(55 cases)received early extensor rehabilitation.The recovery of upper limb motor ability,ultrasonic muscle structure parameters,upper extremity electromyographic signal,self-care ability of daily life and quality of life were compared.Result:After treatment,the score of Fugl-Meyer Assessment(FMA)in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the muscle fiber length and thickness of biceps in the study group were higher than those in the control group,and the echo intensity was lower than that in the control group,with statistical significance(P<0.05).After treatment,the root mean square value of biceps and triceps and the synergistic contraction rate during elbow extension and elbow flexion were greater than those of the control group,with a statistically significant difference(P<0.05).After treatment,the score of Barthel Index(BI)of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of SF-36 in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Early extensor muscle rehabilitation can promote the recovery of hemiplegic upper limb motor ability,improve ultrasonic muscle structure,electromyographic signal,improve daily life self-care ability and quality of life in patients with cerebral infarction during recovery.
