摘  要:目的:调查汉川市幼儿园、小学儿童维生素D营养状况,分析其维生素D缺乏的影响因素。方法:选取2024年1月9—11日1所幼儿园及2所小学就读的236名2~12岁儿童作为研究对象,采用化学发光免疫分析法检测其血清25-羟维生素D[25-(OH)D]水平,采用院内自制问卷收集儿童一般资料,采用多因素logistic回归分析维生素D缺乏的影响因素。结果:236例儿童25-(OH)D水平为(25.63±4.22)ng/mL,处于中低等水平,维生素D缺乏率为20.34%;年龄为2~6岁、每日摄入奶量情况为未摄入及<300 mL、每周未进食动物肝脏或蛋黄、每日未补充维生素D、每日户外活动时间<1 h或1~2 h、贫血及厌食的儿童25-(OH)D水平更低,维生素D缺乏率更高,差异有统计学意义(P<0.05);每日摄入奶量、每周进食动物肝脏或蛋黄、每日补充维生素D及每日户外活动时间为影响儿童25-(OH)D水平的因素,差异有统计学意义(P<0.05)。结论:汉川市幼儿园、小学儿童25-(OH)D水平处于低中等水平,尤其是每日摄入奶量少、每周未进食动物肝脏或蛋黄、每日未补充维生素D及每日户外活动时间少的儿童,其家属需注意补充维生素D,改善其维生素D营养状况,降低维生素D缺乏率。Objective:To investigate the vitamin D nutritional status of kindergarten and primary school children in Hanchuan city,and analyze the influencing factors of vitamin D deficiency.Method:236 children aged 2-12 years old from 1 kindergarten and 2 primary schools were selected as the study objects from 9 January,2024 to 1 November,2024,and their serum 25-hydroxyvitamin D[25-(OH)D]levels were detected by chemiluminescence immunoassay.General data of the children were collected by self-made questionnaires in the hospital.Multivariate logistic regression was used to analyze the influencing factors of vitamin D deficiency.Result:The 25(OH)D level in 236 children was(25.63±4.22)ng/mL,which was at a moderate to low level.The vitamin D deficiency rate was 20.34%.Children aged 2-6 years old,with daily milk intake of less than 300 mL,no animal liver or egg yolk intake per week,no daily supplementation of vitamin D,less than 1 hour or 1-2 hours of outdoor activity per day,anemia and anorexia,have lower levels of 25-(OH)D and higher rates of vitamin D deficiency,and the differences were statistically significant(P<0.05).The influencing factors of 25-(OH)D levels in children with daily milk intake,weekly consumption of animal liver or egg yolks,daily supplementation of vitamin D,and daily outdoor activity time(P<0.05).Conclusion:The 25-(OH)D levels of children in kindergartens and primary schools in Hanchuan City are at a low to moderate level,especially those who consume less milk per day,do not eat animal liver or egg yolks per week,do not supplement vitamin D daily,and have less outdoor activities per day.Their families should pay attention to vitamin D supplementation for these children,improve their vitamin D nutritional status,and reduce their vitamin D deficiency rate.
