不同手术切口联合保留耳大神经对腮腺良性肿瘤患者治疗效果及美观满意度的影响观察

摘　　要：目的探讨不同手术切口联合保留耳大神经对腮腺良性肿瘤患者治疗效果及美观满意度的影响。方法选取106例腮腺良性肿瘤患者为研究对象,随机分为对照组和研究组,各53例。对照组采取S形切口术式联合保留耳大神经治疗,研究组采取V形切口术式联合保留耳大神经治疗。比较两组治疗相关指标(手术时长、切口长度、失血量、引流量、住院时长),应激反应指标[总抗氧化能力(TAC)、皮质醇(Cor)、去甲肾上腺素(NE)]水平,并发症发生情况,美观满意度。结果研究组手术时长(90.97±8.39)min长于对照组的(82.45±7.24)min,切口长度(5.98±0.56)cm小于对照组的(9.64±1.17)cm,失血量(36.05±5.01)ml、引流量(37.45±4.56)ml少于对照组的(43.15±7.33)、(41.79±6.45)ml,住院时长(5.71±0.95)d短于对照组的(6.34±1.10)d(P<0.05)。术后,两组TAC、Cor、NE较术前增高,但研究组TAC(13.88±2.45)kIU/L、Cor(259.27±21.63)μg/L、NE(290.44±17.05)ng/L均低于对照组的(18.64±3.17)kIU/L、(290.44±26.65)μg/L、(320.84±20.41)ng/L(P<0.05)。研究组并发症发生率(3.77%)较对照组(15.09%)低(P<0.05)。研究组美观总满意度(94.34%)高于对照组(79.25%)(P<0.05)。结论采取V形切口术式联合保留耳大神经治疗腮腺良性肿瘤效果优于S形切口,可减少创伤,减轻手术操作所致应激反应程度,利于降低并发症发生风险,且患者美观满意度较高。Objective To explore the effect of different surgical incisions combined with preservation of the great auricular nerve on the treatment effect and aesthetic satisfaction of patients with benign parotid tumors.Methods A total of 106 patients with benign parotid tumors were selected as the study subjects and randomly divided into control group and study group,with 53 cases in each group.The control group was treated with S-shaped incision combined with preservation of the great auricular nerve,and the study group was treated with V-shaped incision combined with preservation of the great auricular nerve.Comparison of the treatment related index(operation duration,incision length,blood loss,drainage volume,length of hospital stay),stress response index[total antioxidant capacity(TAC),cortisol(Cor),norepinephrine(NE)]levels,complications and aesthetic satisfaction between the two groups.Results The study group had a longer operation duration of(90.97±8.39)min than(82.45±7.24)min in the control group,a smaller incision length of(5.98±0.56)cm than(9.64±1.17)cm in the control group;the study group had blood loss of(36.05±5.01)ml and drainage volume of(37.45±4.56)ml,which were lower than(43.15±7.33)and(41.79±6.45)ml in the control group;the study group had shorter length of hospital stay of(5.71±0.95)d than(6.34±1.10)d in the control group(P<0.05).After operation,TAC,Cor and NE in both group were higher than those before operation;the study group had TAC of(13.88±2.45)kIU/L,Cor of(259.27±21.63)μg/L and NE of(290.44±17.05)ng/L,which were lower than(18.64±3.17)kIU/L,(290.44±26.65)μg/L and(320.84±20.41)ng/L in the control group(P<0.05).The complication rate of the study group(3.77%)was lower than that of the control group(15.09%)(P<0.05).The aesthetic satisfaction of the study group(94.34%)was higher than that of the control group(79.25%)(P<0.05).Conclusion The effect of V-shaped incision combined with the preservation of the great auricular nerve in the treatment of benign parotid tumors is better than th
