摘　　要：目的探讨慢性萎缩性胃炎(CAG)患者血清胃泌素(GAS)、内皮素(ET)、白细胞介素-1β(IL-1β)水平变化及临床意义。方法选取2020年10月至2022年10月郑州大学附属郑州中心医院收治的102例CAG患者为CAG组,并纳入同期进行健康体检的健康患者98例为对照组,检测两组血清GAS、ET、IL-1β水平。根据胃黏膜组织病理活检结果,将CAG组分为轻度CAG组(31例)、中度CAG组(42例)和重度CAG组(29例),比较各组血清GAS、ET、IL-1β水平,采用受试者工作特征(ROC)曲线分析血清GAS、ET、IL-1β对CAG患者胃黏膜损伤程度的诊断价值。结果CAG组血清ET、IL-1β水平均高于对照组,GAS水平低于对照组,差异有统计学意义(P<0.05);不同胃黏膜损伤分组间血清ET、IL-1β水平比较,轻度CAG组<中度CAG组<重度CAG组,不同胃黏膜损伤分组间血清GAS水平比较,轻度CAG组>中度CAG组>重度CAG组,差异有统计学意义(P<0.05);血清GAS、ET、IL-1β三者联合ROC曲线下面积(AUC)高于单独检测,差异有统计学意义(P<0.05)。结论CAG患者伴随血清GAS水平降低,血清ET、IL-1β水平升高,而胃黏膜损伤加重。Objective To explore the level changes and clinical significance of serum gastrin(GAS),endothelin(ET)and interleukin-1β(IL-1β)in patients with chronic atrophic gastritis(CAG).Methods A total of 102 patients with CAG and 98 healthy controls in the Zhengzhou Central Hospital Affiliated to Zhengzhou University were enrolled as CAG group and control group between October 2020 and October 2022,respectively.The levels of serum GAS,ET and IL-1βin the two groups were detected.According to pathological biopsy results of gastric mucosa,patients in CAG group were divided into mild CAG group(31 cases),moderate CAG group(42 cases)and severe CAG group(29 cases),and levels of serum GAS,ET and IL-1βin the three groups were compared.The diagnostic value of serum GAS,ET and IL-1βfor the severity of gastric mucosal injury in CAG patients was analyzed by receiver operating characteristic(ROC)curves.Results The levels of serum ET and IL-1βin CAG group were higher than those in control group,while GAS level was lower than that in control group(P<0.05).The levels of serum ET and IL-1βwere the highest in severe CAG group,followed by moderate CAG group and mild CAG group,while GAS level was on the contrary(P<0.05).The area under the curve(AUC)of serum GAS combined with ET and IL-1βwas greater than that of single index(P<0.05).Conclusion In CAG patients,level of serum GAS is decreased,levels of serum ET and IL-1βare increased,and severity of gastric mucosal injury is aggravated.
