利妥昔单抗与吗替麦考酚酯治疗视神经脊髓炎的效果比较

摘　　要：目的对比利妥昔单抗(RTX)与吗替麦考酚酯(MMF)治疗视神经脊髓炎(NMO)患者的效果。方法回顾性选择2020年1月至2022年10月于南阳市中心医院接受治疗的69例NMO患者为研究对象,并按治疗方法分成RTX组(37例)和MMF组(32例),治疗和随访时长为6个月。比较两组视力情况[平均最小分辨角对数(logMAR)矫正视力、视野平均缺损度(MD)、平均敏感度(MS)]、扩展残疾状态量表(EDSS)评分、疾病复发情况和生存质量(MSIS-29)评分的差异。结果两组治疗后的平均IogMAR矫正视力、MS均高于同组治疗前,MD、EDSS各维度评分均低于同组治疗前(P<0.05),但组间差异无统计学意义(P>0.05);两组治疗后平均复发次数低于同组治疗前,且RTX组低于MMF组(P<0.05);两组治疗后的MSIS-29评分均低于同组治疗前,且RTX组的生存质量改善程度优于MMF组(P<0.05)。结论RTX和MMF均在NMO患者的治疗中发挥了显著的效果,其在视力水平、疾病残疾改善程度方面的作用均无明显差异,但RTX更有利于降低患者的复发率,提高生活质量。Objective To compare and analyze the curative effect of rituximab(RTX)and mycophenolate mofeti(MMF)in patients with neuromyelitis optica(NMO).Methods A total of 69 patients with NMO treated in Nanyang Central Hospital were retrospectively enrolled as the research objects from January 2020 to October 2022.According to treatment methods,they were divided into RTX group(37 cases)and MMF group(32 cases).All were treated and followed up for 6 months.The differences in visual acuity[mean logMAR corrected visual acuity,mean defect(MD),mean sensitivity(MS)],score of expanded disability status scale(EDSS)score,annual recurrence rate and score of multiple sclerosis impact scale(MSIS-29)were compared between the two groups.Results After treatment,mean logMAR corrected visual acuity and MS were increased,while MD and EDSS scores were decreased in both groups(P<0.05).But there was no statistically significant difference between the groups(P>0.05).After treatment,mean recurrence frequency was decreased in both groups,which was lower in RTX group than MMF group(P<0.05).After treatment,MSIS-29 scores were decreased in both groups,and the improvement of quality of life was better in RTX group than MMF group(P<0.05).Conclusion The curative effect of both RTX and MMF is significant in NMO patients,and there is no significant difference in the improvement of visual acuity and disease disability.However,RTX is more beneficial to reduce the recurrence rate and improve quality of life.
