摘  要:目的 探讨血清中枢神经特异性蛋白(S100β)、神经元特异性烯醇化酶(NSE)、甲状腺相关激素水平与急性脑梗死后抑郁的关系。方法 回顾性收集100例急性脑梗死患者的临床资料,根据急性脑梗死患者抑郁发生情况分为研究组(有抑郁症状)31例与对照组(无抑郁症状)69例。比较两组患者血清神经生化标志物(S100β、NSE)和甲状腺相关激素[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)]水平;采用Pearson法分析血清S100β、NSE与TSH、FT3、FT4水平与急性脑梗死患者抑郁的相关性;并绘制受试者工作特征(ROC)曲线评估血清S100β、NSE与TSH、FT3、FT4水平对急性脑梗死患者抑郁的预测价值。结果 研究组患者血清S100β、NSE、TSH水平高于对照组,FT3、FT4水平低于对照组(P<0.01)。Pearson相关性分析显示,急性脑梗死后抑郁患者HAMD-17评分与血清S100β、NSE及TSH水平呈正相关,与FT3、FT4水平呈负相关(P<0.01)。ROC曲线分析显示,血清S100β、NSE、TSH、FT3、FT4水平对急性脑梗死患者抑郁均具有较好预测价值,其中FT4的预测价值相对更好(P<0.01)。结论 急性脑梗死后抑郁患者血清S100β、NSE、FT4水平出现异常升高,而TSH、FT3水平出现异常降低,可通过观察上述指标变化早期识别急性脑梗死后抑郁。Objective To explore the relationship of serum central nervous system-specific protein(S100β),neuron-specific enolase(NSE)and thyroid hormones level with depression following acute cerebral infarction.Methods The clinical data of 100 patients with acute cerebral infarction were retrospectively collected and they divided into research(with depressive symptoms,n=31)and control(without,n=69)group according to the occurrence of depression.The levels of serum neurobiochemical ma rkers S100β,NSE and thyroid related hormones(thy roid-st imulat ing hormone[TSH],f ree triiodothyronine[FT3]and free thyroxine[FT4])were compared between the two groups.Pearson method was used to analyze the correlation of serum S100β,NSE,TSH,FT3 and FT4 level with depression following acute cerebral infarction.Receiver operating characteristic(ROC)curves were plotted to assess the predictive value of serum S100β,NSE,TSH,FT3 and FT4 level for depression following acute cerebral infarction.Results Serum S100β,NSE and TSH level were higher and FT3 and FT4 lower in research than control group(P<0.01).Pearson correlation analysis showed that the 17-item Hamilton Depression Rating Scale scores were positively correlated with serum S100β,NSE and TSH levels and negatively with FT3 and FT4 in patients with acute cerebral infarction(P<0.01).ROC curve analysis showed that serum S100β,NSE,TSH,FT3 and FT4 level had good predictive values for depression following acute cerebral infarction and FT4 was better(P<0.01).Conclusion Serum S100β,NSE and FT4 level abnormally increase and TSH and FT3 decrease in patients with acute cerebral infarction,depression following acute cerebral infarction could be early recognized via observing the changes of above indicators.
