摘 要:目的探讨动态脑电图(VEEG)联合CT对缺血缺氧性脑病(HIE)患儿早期诊断的临床价值。方法选取2020年7月至2022年12月景德镇市第三人民医院儿科收治的100例疑似HIE住院患儿作为研究对象,按照随机数字表法分为对照组与试验组,每组50例。对照组采用CT检查方法,试验组采用VEEG联合CT检查方法。比较两组患儿的检查与HIE临床分度诊断符合率、Apgar评分、检查颅内出血率情况以及出生后1、3、6个月的听力、视力以及智力情况。结果试验组准确度高于对照组,差异有统计学意义(P<0.05)。两组的灵敏度和特异度比较,差异无统计学意义(P>0.05);试验组诊断总符合率高于对照组,差异有统计学意义(P<0.05)。出生1、5 min,试验组的Apgar评分高于对照组,差异有统计学意义(P<0.05)。两组患儿检查各方面的颅内出血率比较,差异无统计学意义(P>0.05)。出生后1、3、6个月,两组患儿听阈、视力及智力的时间、组间、交互作用比较,差异无统计学意义(P>0.05)。结论VEEG联合CT检查能够较早发现HIE患儿脑功能异常,给予HIE患儿早期干预,动态调整优化诊疗方案,有助于降低新生儿的致残率以及死亡率。Objective To investigate the clinical value of video electroencephalography(VEEG)combined with CT in early diagnosis of hypoxic ischemic encephalopathy(HIE).Methods A total of 100 hospitalized children with suspected HIE admitted to the Third People's Hospital of Jingdezhen from July 2020 to December 2022 were selected as the study objects,and they were divided into control group and experimental group according to random number table method,with 50 cases in each group.The control group was examined by CT,and the experimental group was examined by VEEG combined with CT.The coincidence rate of HIE clinical grade diagnosis,Apgar score,intracranial hemorrhage rate,hearing,vision and intelligence at 1,3 and 6 months after birth were compared between the two groups.Results The accuracy of experimental group was higher than that of control group,and the difference was statistically significant(P<0.05).There were no significant differences in sensitivity and specificity between the two groups(P>0.05).The total coincidence rate of diagnosis of experimental group was higher than that of control group,and the difference was statistically significant(P<0.05).At 1 and 5 min after birth,the Apgar scores of experimental group were higher than those of control group,and the differences were statistically significant(P<0.05).There was no significant difference in the rate of intracranial hemorrhage between the two groups(P>0.05).At 1,3 and 6 months after birth,there were no significant differences between the two groups in auditory threshold,visual acuity and intelligence(P>0.05).Conclusion VEEG combined with CT examination can detect brain function abnormalities in children with HIE early,and provide early intervention to children with HIE and dynamically adjust and optimize the diagnosis and treatment plan,which is helpful to reduce the disability rate and mortality rate of newborns.
