摘　　要：【目的】探讨多发性骨髓瘤(MM)患者C反应蛋白(CRP)、乳酸脱氢酶(LDH)及β_(2)-微球蛋白(β_(2)-MG)水平与预后的关系。【方法】对96例MM患者的临床资料行回顾性分析,统计患者年龄、性别、国际分期(ISS)、免疫球蛋白分型、合并疾病、凝血指标、血脂指标、同型半胱氨酸(Hcy)及CRP、LDH、β_(2)-MG水平等相关资料,分析影响MM患者预后的相关因素,采用Spearman检验分析患者Hcy、CRP、LDH及β_(2)-MG与预后的相关性。【结果】96例患者中,有79例(82.29%)患者存活,纳为存活组;有17例(17.71%)患者死亡,纳为死亡组。死亡组ISS分期为Ⅱ、Ⅲ期所占比例及CRP、LDH、β_(2)-MG、Hcy水平均高于存活组(P<0.05)。CRP、LDH、β_(2)-MG、Hcy水平是MM患者预后的影响因素(P<0.05)。Spearman检验显示,MM患者的Hcy、CRP、LDH及β_(2)-MG水平与预后呈正相关(P<0.05)。【结论】随着Hcy、CRP、LDH及β_(2)-MG水平升高,患者病情加重,其预后不良的风险发生率提升。【Objective】To explore the relationship of prognosis with C-reactive protein(CRP),lactate dehydrogenase(LDH),andβ_(2)-microglobulin(β_(2)-MG)levels in multiple myeloma(MM)patients.【Methods】A retrospective analysis was conducted on the clinical data of 96 MM patients,including age,gender,International Staging System(ISS)stage,immunoglobulin subtype,comorbidities,coagulation markers,lipid profiles,homocysteine(Hcy)levels,and CRP,LDH,andβ_(2)-MG levels.Factors influencing prognosis were analyzed,and Spearman's correlation test was used to analyze the correlation of prognosis with levels of Hcy,CRP,LDH andβ_(2)-MG.【Results】Among the 96 patients,79(82.29%)were alive and were categorized into the survival group,while 17(17.71%)died and were categorized into the death group.The proportion of ISS stageⅡandⅢ,as well as CRP,LDH,β_(2)-MG,and Hcy levels in the death group,were significantly higher than those in the survival group(P<0.05).CRP,LDH,β_(2)-MG,and Hcy were found to be significant prognostic factors for MM(P<0.05).Spearman's test showed that Hcy,CRP,LDH,andβ_(2)-MG levels were positively correlated with prognosis(P<0.05).【Conclusion】As the levels of Hcy,CRP,LDH,andβ_(2)-MG increase,the severity of the disease worsens,and the risk of poor prognosis in MM patients rises.
