摘 要:目的探讨鼻腔鼻窦内翻性乳头状瘤(SNIP)组织中生存素、缺氧诱导因子-1α(HIF-1α)、细胞角蛋白10(CK10)表达量与恶变风险的关系及预警价值。方法选取太原市中心医院2019年1月至2023年8月收治的188例SNIP患者作为研究对象,根据是否发生恶变分为SNIP组(153例)与内翻性乳头状瘤恶变(IP-SCC)组(32例),比较两组患者的临床资料、生存素、HIF-1α、CK10表达情况,分析生存素、HIF-1α、CK10对恶变风险预警价值。结果Ridit检验显示,IP-SCC组患者的生存素、HIF-1α、CK10表达强度高于SNIP组(P<0.05);χ^(2)检验显示,IP-SCC组生存素、HIF-1α、CK10阳性表达率高于SNIP组(P<0.05);校正其他因素前后,生存素、HIF-1α、CK10阳性表达均是SNIP恶变风险的危险因素(P<0.05);生存素、HIF-1α、CK10预测SNIP恶变风险的曲线下面积(AUC)分别为0.724、0.711、0.764,均具有预测效能(P<0.05),联合预测SNIP恶变风险的AUC为0.920,预警价值明显优于各指标单独预警价值(P<0.05)。结论SNIP组织中生存素、HIF-1α、CK10表达强度与恶变风险密切相关,且阳性表达是恶变风险的独立危险因素,联合检测具有较高的预警价值。Objective To investigate the relationship between the expression levels of survivin,hypoxia-inducible factor-1α(HIF-1α),and cytokeratin 10(CK10)in tissues of sinonasal inverted papilloma(SNIP)and their malignant transformation risk,as well as their predictive value.Methods A total of 188 SNIP patients from Taiyuan Central Hospital from January 2019 to August 2023 were selected and divided into the SNIP group and the inverted papilloma malignant transformation(IP-SCC)group based on whether malignant transformation occurred.The clinical data,as well as the expression levels of Survivin,HIF-1α,and CK10,were compared between the two groups.The predictive value of Survivin,HIF-1α,and CK10 for malignant transformation risk was analyzed.Results The Ridit test revealed that the expression intensity of Survivin,HIF-1α,and CK10 in the IP-SCC group was higher than that in the SNIP group(P<0.05).Theχ^(2) test indicated that the positive expression rates of Survivin,HIF-1α,and CK10 in the IP-SCC group were higher than those in the SNIP group(P<0.05).Both before and after adjusting for other factors,the positive expressions of Survivin,HIF-1α,and CK10 were independent risk factors for the malignant transformation risk of SNIP(P<0.05).The AUC values for predicting the malignant transformation risk of SNIP using Survivin,HIF-1α,and CK10 were 0.724,0.711,and 0.764,respectively.The combined prediction of these markers yielded an AUC of 0.920,demonstrating significantly superior predictive value compared to each individual marker alone(P<0.05).Conclusion The expression intensity of Survivin,HIF-1α,and CK10 in SNIP tissues was significantly linearly correlated with the risk of malignant transformation,and positive expression was an independent risk factor for malignant transformation.Combined detection had high early warning value.
