懒惰狡诈球菌感染导致睑缘炎的病例报告1例

摘　　要：懒惰狡诈球菌是一种兼性厌氧革兰氏阳性菌,是上呼吸道菌群的一部分,可引起肺部感染、败血症、胆囊炎和滑膜炎。在眼科方面已经有数例报道其引起的角膜炎、结膜炎。本文描述1例由懒惰狡诈球菌引起的睑缘炎,并系统回顾全球已发表的由该菌引起的眼部相关报道。本病例的确诊方法为新一代宏基因组测序(mNGS)检查,体现了mNGS具有促进快速识别难以诊断的眼部感染的能力。在给予万古霉素静脉滴注,万古霉素滴眼液(自配)、左氧氟沙星眼膏治疗后,炎症好转,分泌物明显减少。后期复诊时没有分泌物出现,炎症得到基本控制。本例因懒惰狡诈球菌感染导致睑缘炎的病例,为其他研究人员和临床医师提供了治疗该微生物感染的参考。Dolosigranulum pigrum is a facultative anaerobic gram-positive bacterium that forms part of the upper respiratory tract flora and can cause lung infections,sepsis,cholecystitis,and synovitis.Several cases of keratitis and conjunctivitis caused by it have been reported in ophthalmology.This article will describe a case of blepharitis caused by Dolosigranulum pigrum and review the worldwide published case reports of ocular infections caused by it.The diagnosis of this case was confirmed by metagenomic next-generation sequencing(mNGS),demonstrating the ability of mNGS to facilitate rapid identification of difficult eye infections.After treatment with intravenous administration of Vancomycin,Vancomycin eye drops(self-administered),and Levofloxacin eye ointment,the inflammation improved and the discharge decreased significantly.There was no discharge continued to appear at follow-up visit and inflammation was basically controlled.This is the first case of blepharitis due to Dolosigranulum pigrum infection in China,providing a reference for other researchers and clinicians to treat this microbial infection.
