摘 要:目的评估全身炎症指数与第二眼白内障术后疼痛发生之间的关联,并检测其作为诊断标志物的潜力。方法本研究采用前瞻性病例对照研究设计,选取2023年1月~2024年1月在哈尔滨医科大学附属第二医院眼科计划行白内障手术的患者80例,其中初次行白内障手术的患者40例(第一眼组),完成一眼白内障手术后3月内行另一眼手术的患者40例(第二眼组)。全身炎症指标,如中性粒细胞与淋巴细胞比值(NLR)、血小板淋巴细胞比值(PLR)、单核细胞与淋巴细胞比值(MLR)和全身免疫炎症指数(SII),均通过血常规结果计算得出,而术后疼痛通过视觉模拟评分法(VAS)检测。比较两组之间炎症指标及VAS的差异。结果两组患者一般情况、人口统计学资料及白内障超声乳化手术资料差异无统计学意义(P>0.05)。第二眼组的NLR、MLR较第一眼组,均显著提高[(1.94±0.71)vs.(1.60±0.74)、(0.28±0.13)vs.(0.23±0.11)],差异有统计学意义(t=2.289、2.329,P<0.05);两组患者PLR及SII差异无统计学意义(P>0.05)。第二眼组的VAS(1.80±1.47)分较第一眼组(0.79±0.95)分显著提高,差异有统计学意义(t=2.674,P=0.001);且NLR、MLR均与VAS呈正相关(r=0.538、0.377,P<0.05)。此外,ROC分析发现,NLR及MLR均可作为第二眼白内障术后疼痛有效诊断标志物(AUC=0.8259、0.816,P<0.05)。结论第二眼白内障患者中发现的较高NLR及MLR水平与术后疼痛相关,具有作为诊断标志物的潜力,需进一步研究证实相关结论。Objective To evaluate the association between systemic inflammation index and postoperative pain in second eye cataract surgery,and to assess its potential as a diagnostic biomarker.Methods This study adopted a prospective case-control design and included 80 patients who underwent planned cataract surgery at the ophthalmology department of the Second Affiliated Hospital of Harbin Medical University from January 2023 to January 2024.Among them,40 patients underwent cataract surgery for the first time(first eye group)and 40 patients underwent cataract surgery for the other eye within 3 months after completing one eye surgery(second eye group).The systemic inflammatory indicators,including neutrophil-to-lymphocyte ratio(NLR),platelet lymphocyte ratio(PLR),monocyte-to-lymphocyte ratio(MLR),and systemic immune-inflammation index(SII),were calculated based on blood routine results,while postoperative pain was detected using visual analog scale(VAS).Differences in inflammatory markers and VAS between the two groups were compared.Results There were no significant differences in general conditions,demographic data,and cataract phacoemulsification surgery data between the first eye group and the second eye group(P>0.05).The NLR and MLR of the second eye group were significantly higher than those of the first eye group(t=2.289,2.329;P<0.05).The VAS of the second eye group(1.80±1.47)was significantly higher than that of the first eye group(0.79±0.95)(t=2.674;P=0.001),and both NLR and MLR were correlated with VAS(r=0.538,0.377;P<0.05).In addition,ROC analysis found that both NLR and MLR can serve as effective diagnostic biomarkers for postoperative pain in second eye cataract surgery(AUC=0.8259,0.816;P<0.05)。Conclusion The higher levels of NLR and MLR found in patients with second eye cataracts are associated with postoperative pain and have the potential to serve as diagnostic biomarkers.Further research is needed to confirm the relevant conclusions.
