摘  要:目的了解胃癌术后患者营养知信行现状,探索营养知信行(KAP)的影响因素及其与生活质量的相关性。方法选取2023年6月至2024年4月南华大学附属第一医院收治的180例胃癌患者进行问卷调查,包括一般资料、消化系统肿瘤患者营养知识-态度-行为问卷和癌症治疗功能评价普适性量表,采用多元线性回归分析营养知信行的影响因素,采用Pearson检验分析营养KAP与生活质量的相关性。结果胃癌术后患者营养KAP总分为(43.94±5.63)分,生活质量总分为(66.67±8.18)分;多因素分析结果提示,男性(β=0.333,B=2.795,95%CI:2.758~5.414)、老年(β=0.241,B=2.795,95%CI:1.524~4.066)、文化程度低(β=0.426,B=2.592,95%CI:1.907~3.277)是营养KAP得分低的危险因素;KAP总分、各维度与生活质量总分、各维度之间呈正相关(r=0.201~0.840),差异有统计学意义(P<0.01)。结论胃癌术后患者营养KAP和生活质量处于中等偏下水平,并且存在正相关,营养教育过程中应充分考虑患者的性别、年龄、文化程度等因素,采取个性化的营养干预指导,通过提升营养KAP水平改善患者的生活质量。Objective To comprehend the current nutritional knowledge,attitude,belief and practice(KAP)of postoperative gastric cancer patients,investigate the influencing factors of these aspects,and to assess their correlation with quality of life.Methods A questionnaire survey was conducted among 180 gastric cancer patients admitted to the First Affiliated Hospital of University of South China from June 2023 to April 2024,encompassing general information,digestive cancer patients nutrition knowledge,attitude and practice questionnaire and functional assessment of cancer therapy.Multiple linear regression was utilized to analyze the influencing factors of nutritional KAP,while the Pearson test was employed to explore the correlation between nutritional KAP and quality of life.Results The total score of nutritional KAP for postoperative gastric cancer patients was(43.94±5.63)points,and the total score of quality of life was(66.67±8.18)points.The results of the multivariate analysis indicated that male(β=0.333,B=2.795,95%CI:2.758-5.414),elderly(β=0.241,B=2.795,95%CI:1.524-4.066),low literacy(β=0.426,B=2.592,95%CI:1.907-3.277)were risk factors for low scores in nutritional KAP.A positive correlation was found between the total score and dimensions of nutritional KAP,as well as the total score and dimensions of quality of life(r=0.201-0.840),with statistically significant differences(P<0.01).Conclusion The nutritional KAP and quality of life of postoperative gastric cancer patients are at a moderate to low level,and there is a positive correlation between these factors.In nutrition education,it is essential to consider patients'gender,age,and educational level to provide personalized nutritional intervention guidance,aiming to enhance their nutritional KAP,thereby improving their quality of life.
