摘　　要：目的探讨腹腔镜下膀胱癌根治术患者淋巴结清扫时间的影响因素。方法根据淋巴结清扫时间的不同将80例膀胱癌根治术患者分为长时间组(n=33,淋巴结清扫时间﹥34 min)和短时间组(n=47,淋巴结清扫时间≤34 min)。比较两组患者的临床资料,采用Logistic回归模型分析腹腔镜下膀胱癌根治术患者淋巴结清扫时间的影响因素。比较两组患者的手术相关指标及术后并发症发生情况。结果单因素及多因素分析结果显示,性别、体重指数、术者手术经验均是腹腔镜下膀胱癌根治术患者淋巴结清扫时间的影响因素(P﹤0.05)。长时间组患者术中出血量明显多于短时间组,住院时间明显长于短时间组,差异均有统计学意义(P﹤0.01)。两组患者的并发症总发生率比较,差异无统计学意义(P﹥0.05)。结论性别、体重指数、术者手术经验均是腹腔镜下膀胱癌根治术患者淋巴结清扫时间的影响因素,患者为女性、肥胖及术者手术经验﹤30例均可能导致淋巴结清扫时间延长,造成术中出血量增加,住院时间延长。Objective To explore the influencing factors of lymph node dissection time in patients undergoing laparoscopic radical surgery for bladder cancer.Method According to the different lymph node dissection time,80 patients who underwent laparoscopic radical surgery for bladder cancer were divided into long time group(n=33,lymph node dissection time>34 min)and short time group(n=47,lymph node dissection time≤34 min).The clinical data of the two groups were compared.Logistic regression model was used to analyze the influencing factors of lymph node dissection time in patients undergoing laparoscopic radical surgery for bladder cancer.The surgery-related indexes and postoperative complications were compared between the two groups.Result The results of univariate and multivariate analysis showed that gender,body mass index and surgical experience of surgeon were influencing factors of lymph node dissection time in patients undergoing laparoscopic radical surgery for bladder cancer(P<0.05).The amount of intraoperative blood loss in the long time group was significantly higher than that in the short time group,and the hospitalization time was significantly longer than that in the short time group,the differences were statistically significant(P<0.01).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Gender,body mass index and surgical experience of surgeon are influencing factors of lymph node dissection time in patients undergoing laparoscopic radical surgery for bladder cancer.The patient is female,obese and the surgical experience of surgeon is less than 30 cases may lead to prolonged lymph node dissection time,and then increase intraoperative blood loss and prolong hospitalization time.
