摘　　要：目的探讨改良前列腺尖部分离技术对腹腔镜前列腺癌根治术(LRP)患者术后并发症及尿控功能的影响。方法根据手术方式的不同将80例前列腺癌患者分为对照组(n=39,常规LRP手术)和观察组(n=41,改良前列腺尖部分离技术辅助LRP手术)。比较两组患者的手术相关指标、尿控功能[扩展前列腺癌复合指数(EPIC-UIN)、国际尿失禁咨询委员会尿失禁问卷简表(ICIQ-SF)]及术后并发症发生情况。结果观察组患者尿管留置时间和住院时间均明显短于对照组,差异均有统计学意义(P﹤0.01)。术后1个月,两组患者ICIQ-SF评分均低于本组术前,EPIC-UIN评分均高于本组术前,观察组患者ICIQ-SF评分低于对照组,EPIC-UIN评分高于对照组,差异均有统计学意义(P﹤0.05)。观察组患者术后并发症总发生率低于对照组,差异有统计学意义(P﹤0.05)。结论改良前列腺尖部分离技术辅助LRP手术治疗前列腺癌患者可减少并发症,提高尿控功能,促进患者术后恢复。Objective To investigate the effect of modified prostate apex separation technique on postoperative complications and urinary control function in patients undergoing laparoscopic radical prostatectomy(LRP).Method According to different surgical methods,80 patients with prostate cancer were divided into control group(n=39,received conventional LRP surgery)and observation group(n=41,received modified prostate apex separation technique assisted LRP surgery).The surgery-related indicators,urinary control function[expanded prostate cancer index composite-urinary inventory(EPIC-UIN),International Consultation on Incontinence questionnaire short-form(ICIQ-SF)]and postoperative complications were compared between the two groups.Result The urinary catheter indwelling time and hospitalization time in observation group were significantly shorter than those in control group,and the differences were statistically significant(P<0.01).One month after surgery,the ICIQ-SF scores in both groups were lower than those before surgery,the EPIC-UIN scores were higher than those before surgery,and the ICIQ-SF score in observation group was lower than that in control group,the EPIC-UIN score was higher than that in control group,the differences were statistically significant(P<0.05).The total incidence of postoperative complications in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion The modified prostate apex separation technique assisted LRP surgery can reduce complications,improve urinary control function and promote postoperative recovery of patients with prostate cancer.
