摘 要:目的探讨超声血流参数对宫颈癌的诊断价值及与临床分期的关系。方法选取106例宫颈疾病患者,均接受经阴道超声检查。术后病理检查显示,宫颈癌54例,子宫良性病变52例,比较宫颈癌与子宫良性病变患者、不同临床分期宫颈癌患者的超声血流参数[阻力指数(RI)、舒张末期流速(EDV)、收缩期峰值流速(PSV)]。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估超声血流参数对宫颈癌的诊断价值;超声血流参数与宫颈癌患者临床分期的相关性采用Kendall’s tau-b相关分析。结果宫颈癌患者RI明显低于子宫良性病变患者,EDV、PSV均明显高于子宫良性病变患者,差异均有统计学意义(P﹤0.01)。ROC曲线显示,RI、EDV、PSV联合检测诊断宫颈癌的AUC为0.945(95%CI:0.902~0.987),高于各指标单独检测,此时的灵敏度为0.907,特异度为0.885。随着临床分期的升高,RI逐渐降低,EDV、PSV逐渐升高,差异均有统计学意义(P﹤0.05)。Kendall’s tau-b相关分析结果显示,RI与宫颈癌患者临床分期呈负相关(r=-0.869,P﹤0.01),EDV、PSV与临床分期均呈正相关(r=0.915、0.878,P﹤0.01)。结论超声血流参数对宫颈癌有一定的诊断价值,且RI与宫颈癌患者临床分期呈负相关,EDV、PSV与临床分期均呈正相关。Objective To analyze the diagnostic value of ultrasound blood flow parameters in cervical cancer and their relationship with clinical staging.Method A total of 106 patients with cervical diseases were selected and underwent transvaginal ultrasound examination.Postoperative pathological examination showed 54 cases of cervical cancer and 52 cases of benign uterine diseases.The ultrasound blood flow parameters[resistance index(RI),end diastolic velocity(EDV),peak systolic velocity(PSV)]were compared between patients with cervical cancer and benign uterine diseases,as well as cervical cancer with different clinical stages.The receiver operating characteristic(ROC)curve was drawn,the area under the curve(AUC)was calculated,and the diagnostic value of ultrasound blood flow parameters for cervical cancer was evaluated.The correlation between ultrasound blood flow parameters and clinical staging of cervical cancer was analyzed by Kendall’s tau-b correlation analysis.Result The RI of cervical cancer patients was significantly lower than that in patients with benign uterine diseases,while the EDV and PSV were significantly higher than those of patients with benign uterine diseases,and the differences were statistically significant(P<0.01).The ROC curve showed that the AUC of the RI,EDV,and PSV combined detection in diagnosing cervical cancer was 0.945(95%CI:0.902-0.987),which was higher than single detection of each indicator.At this time,the sensitivity was 0.907 and the specificity was 0.885.As the clinical stage increases,RI gradually decrease,while EDV and PSV gradually increase,and the differences were statistically significant(P<0.05).Kendall’s tau-b correlation analysis showed that RI was negatively correlated with clinical staging of cervical cancer patients(r=-0.869,P<0.01),while EDV and PSV were positively correlated with clinical staging(r=0.915,0.878,P<0.01).Conclusion Ultrasound blood flow parameters combined detection has certain value in diagnosis of cervical cancer,and RI is negatively correlated with c
