摘 要:目的分析CD24在卵巢癌早期诊断和预后预测中的应用价值。方法取80例卵巢癌患者的卵巢癌组织及癌旁正常组织,比较两种组织中CD24表达情况。对所有患者随访2年,根据随访结果分为预后良好和预后不良患者,采用Cox回归模型分析卵巢癌患者预后的影响因素,并比较CD24阳性表达与CD24阴性表达卵巢癌患者的生存情况。结果卵巢癌组织中CD24阳性表达率明显高于癌旁正常组织,差异有统计学意义(P﹤0.01)。预后良好(n=65)和预后不良(n=15)患者CD24表达情况、国际妇产科联盟(FIGO)分期、淋巴结转移情况、人附睾蛋白4(HE4)水平、糖类抗原125(CA125)水平比较,差异均有统计学意义(P﹤0.05)。多因素Cox回归分析结果显示,CD24阳性表达、FIGO分期为Ⅱ期、有淋巴结转移、HE4表达水平≥180.0 pmol/L、CA125表达水平≥35.0 U/ml均是卵巢癌患者预后不良的独立危险因素(P﹤0.05)。CD24阳性表达卵巢癌患者的2年总生存率为74.58%,低于CD24阴性表达卵巢癌患者的100%,差异有统计学意义(P﹤0.05)。结论CD24在卵巢癌组织中的阳性表达率较高,是卵巢癌患者预后不良的独立危险因素,可用于卵巢癌的早期诊断和预后预测。Objective To analyze the application value of CD24 in early diagnosis and prognosis prediction of ovarian cancer.Method Ovarian cancer tissues and adjacent normal tissues from 80 patients with ovarian cancer were collected.The expression of CD24 in the two kinds of tissues was compared.All patients were followed up for 2 years,and divided into good prognosis and poor prognosis patients according to the follow-up results.Cox regression model was used to analyze the influencing factors of prognosis in ovarian cancer patients,and the survival of CD24 positive and CD24 negative ovarian cancer patients was compared.Result The positive expression rate of CD24 in ovarian cancer tissues was significantly higher than that in adjacent normal tissues,and the difference was statistically significant(P<0.01).There were statistically significant differences in CD24 expression,International Federation of Gynecology and Obstetrics(FIGO)stage,lymph node metastasis,human epididymis protein 4(HE4)level,and carbohydrate antigen 125(CA125)level between good prognosis(n=65)and poor prognosis(n=15)patients(P<0.05).The results of multivariate Cox regression analysis showed that CD24 positive expression,FIGO stage II,lymph node metastasis,HE4 expression level≥180.0 pmol/L,and CA125 expression level≥35.0 U/ml were independent risk factors for poor prognosis in patients with ovarian cancer(P<0.05).The 2-year overall survival rate of ovarian cancer patients with CD24 positive expression was 74.58%,which was lower than 100%of patients with CD24 negative expression,and the difference was statistically significant(P<0.05).Conclusion The positive expression rate of CD24 in ovarian cancer tissues is high.It is an independent risk factor for poor prognosis in patients with ovarian cancer and can be used for the early diagnosis and prognosis prediction of ovarian cancer.
