基于品管圈的精细化干预在乳腺癌手术患者中的应用效果

摘　　要：目的探讨基于品管圈的精细化干预在乳腺癌手术患者中的应用效果。方法依据干预方式的不同将124例乳腺癌患者分为对照组(n=63)和观察组(n=61),对照组患者接受常规干预,观察组患者接受基于品管圈的精细化干预。比较两组患者的心理状况[正性负性情绪自评量表(PANAS)]、手术相关指标、并发症发生情况和满意度。结果干预后,两组患者正性情绪评分均高于本组干预前,负性情绪评分均低于本组干预前,观察组患者正性情绪评分高于对照组,负性情绪评分低于对照组,差异均有统计学意义(P﹤0.05)。观察组患者术后肛门排气时间、下床活动时间、导管拔除时间及术后住院时间均明显短于对照组,差异均有统计学意义(P﹤0.01)。观察组患者的并发症总发生率为3.28%,低于对照组患者的14.29%,总满意度为100%,高于对照组患者的88.89%,差异均有统计学意义(P﹤0.05)。结论基于品管圈的精细化干预可改善乳腺癌手术患者的心理状况,促进患者的术后恢复,降低并发症发生风险,提高患者的满意度。Objective To evaluate the application effect of refined intervention based on quality control circle in patients with breast cancer undergoing surgery.Method A total of 124 patients with breast cancer undergoing surgery were divided into control group(n=63)and observation group(n=61)according to the different intervention methods.The patients in the control group received routine intervention,while the patients in the observation group received refined intervention based on the quality control circle.The psychological status[positive and negative affect scale(PANAS)],surgical related indicators,incidence of complications,and satisfaction were compared between the two groups.Result After intervention,the positive emotional scores of both groups were higher than those before intervention,and the negative emotional scores were lower than those before intervention,the positive emotional score in observation group was higher than that in control group,and the negative emotional score was lower than that in control group,and the differences were statistically significant(P<0.05).The postoperative anal exhaust time,ambulation time,catheter removal time,and postoperative hospitalization time in observation group were significantly shorter than those in control group,and the differences were statistically significant(P<0.01).The total incidence of complications in observation group was 3.28%,which was lower than 14.29%in control group,the overall satisfaction rate was 100%,which was higher than 88.89%in control group,and the differences were statistically significant(P<0.05).Conclusion The refined intervention based on quality control circle can improve the psychological state of patients with breast cancer undergoing surgery,promote the postoperative recovery,reduce the risk of complications,and improve satisfaction.
