基于厥阴之象探讨乌梅丸在心律失常中的应用

摘  要:心律失常是临床常见的心血管疾病,以心中悸动、惊惕不安为主要表现。厥阴主症中“气上撞心”一症与心悸相合,而乌梅丸作为厥阴主方,在临床上用于治疗心律失常有可靠疗效,但尚无相关系统论述。故本文立足于厥阴,从厥阴之象出发,认为厥阴在阴阳为阴尽阳生之象,在天为风,在地为木,在人为肝;依厥阴之四象,探讨了厥阴为病而导致心律失常的病机分别为寒热错杂、风胜动心、水火失济及疏泄失职;从而提出了乌梅丸从调寒热、息肝风、济水火、温肝阳四个角度治疗心律失常,以期为心律失常,尤其是复杂型心律失常的中医辨证论治提供新思路。Cardiac arrhythmia is a common clinical cardiovascular disease,with palpitation and agitation in the heart as the main manifestation.The syndrome of“qi hitting the heart”as one of the main syndrome of Jueyin is compatible with palpitations,and Wumei Wan,as the main formula of Jueyin,has been clinically used to treat arrhythmia with reliable efficacy.However,there is no systematic discussion on relevant subject.Therefore,this review is based on the Jueyin and its manifestation.It is believed that Jueyin is where Yin ends and Yang rises,and in heaven it is wind,on the earth it is wood,in human's body it is liver.According to these four signs of Jueyin,the pathogenesis of Jueyin disease leading to cardiac arrhythmias is discussed as coldheat mismatch,wind disturbing the heart,the disharmony of water and fire,and disfunction of liver.Thus,Wumei Wan is proposed to treat palpitations from four angles,namely,regulating cold-heat,calming down liver wind,rebuilding the interaction between water and fire,and warming liver-yang.This review is aimed to provide new ideas for the dialectical treatment of cardiac arrhythmias in Chinese medicine,especially complex arrhythmias.
