益气养血组方对血液恶性肿瘤化疗后骨髓抑制的干预研究

摘　　要：目的探讨益气养血组方对血液恶性肿瘤化疗后骨髓抑制的干预作用及其对患者免疫功能和生活质量的影响。方法采用随机对照研究方法,将2023年7月~2024年10月在山东第二医科大学附属医院及潍坊市中医院接受化疗的180例血液恶性肿瘤患者分为对照组和观察组,每组90例。两组均予常规化疗支持治疗,观察组在此基础上给予益气养血组方(熟地黄15 g、山茱萸12 g、山药15 g、枸杞子10 g等)口服,每日1剂,28 d为一个疗程,共2个疗程。结果与对照组相比,观察组化疗后骨髓抑制的发生率、持续时间,白细胞、血红蛋白、血小板恢复时间等方面有改善,但差异无统计学意义(P>0.05);观察组总有效率高于对照组(P<0.05);观察组CD3^(+)、CD4^(+)T淋巴细胞、NK细胞百分比高,CD8^(+)T淋巴细胞百分比、CD4^(+)/CD8^(+)比值低(P<0.05);观察组IL-2、IL-6水平低,TGF-β1水平高(P<0.05);观察组乏力、气短等中医证候积分及Karnofsky功能状态评分(KPS)优于对照组(P<0.05)。结论益气养血组方能够通过调节免疫功能,改善炎症反应,减轻血液恶性肿瘤患者化疗后骨髓抑制程度,改善患者中医证候和生活质量,提示益气养血组方可作为改善血液肿瘤患者化疗后骨髓抑制和生存质量的有效辅助治疗手段。Objective To explore the interventional effect of Yiqi Yangxue Formula on myelosuppression after chemotherapy in patients with hematologic malignancies and its impact on patients'immune function and quality of life.Methods A randomized controlled trial was conducted.One hundred and eighty patients with hematologic malignancies undergoing chemotherapy at the Affiliated Hospital of Shandong Second Medical University and Weifang Hospital of Traditional Chinese Medicine from July 2023 to October 2024 were randomly divided into a control group and an observation group,with 90 cases in each group.Both groups received conventional chemotherapy and supportive care,and the observation group was additionally given oral Yiqi Yangxue Formula(Rehmannia glutinosa 15 g,Cornus officinalis 12 g,Dioscorea opposita 15 g,Lycium barbarum 10 g,etc.)once daily for 2 courses,28 days per course.Results Compared with the control group,the observation group showed modest improvements in the incidence and duration of post-chemotherapy myelosuppression,as well as in recovery time for white blood cells,hemoglobin,and platelets,although these differences did not reach statistical significance(P>0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).The observation group had higher percentages of CD3^(+),CD4^(+)T lymphocytes and NK cells,and lower percentage of CD8^(+)T lymphocytes and CD4^(+)/CD8^(+)ratio(P<0.05).The levels of IL-2 and IL-6 were lower,while TGF-β1 was higher in the observation group(P<0.05).The TCM syndrome scores of fatigue,shortness of breath,etc.And Karnofsky Performance Status(KPS)scores were better in the observation group than those in the control group(P<0.05).Conclusion Yiqi Yangxue Formula can alleviate the degree of myelosuppression after chemotherapy in patients with hematologic malignancies,improve patients'TCM syndromes and quality of life by regulating immune function and improving inflammatory response,suggesting that Yiqi Yangxue Formula may serve as an effect
