麻黄汤加味治疗中晚期肺癌咳嗽的效果

摘  要:目的观察麻黄汤加味治疗中晚期肺癌咳嗽的效果。方法回顾性分析2021年3月—2024年3月首都医科大学附属北京中医医院肿瘤门诊收治的200例中晚期肺癌咳嗽患者临床资料,按治疗方案不同分为观察组和对照组各100例。观察组给予麻黄汤加味治疗,对照组给予止咳化痰定喘丸治疗。治疗1个月后,比较2组咳嗽缓解情况,治疗前后中医证候积分、炎性因子[C反应蛋白(CRP)、白介素-6(IL-6)]水平、嗜酸性粒细胞百分比(EOS%)变化,不良反应。结果观察组咳嗽总缓解率为95.00%,高于对照组的81.00%(χ^(2)=9.280,P=0.002)。治疗1个月后,2组咳嗽咯痰、喘促、胸闷胸痛、腰膝酸软、神疲乏力、恶心呕吐证候积分,CRP、IL-6水平及EOS%均低于治疗前,且观察组低于对照组(P<0.01)。观察组与对照组不良反应总发生率比较差异无统计学意义(6.00%vs.11.00%,χ^(2)=1.607,P=0.205)。结论麻黄汤加味治疗中晚期肺癌咳嗽的临床效果较好,可明显改善患者的中医证候,缓解炎性反应,且不增加不良反应发生风险,安全性较高。Objective To observe the effect of adding flavor ephedra soup in the treatment of cough in middle and advanced stage lung cancer.Methods Retrospectively selected 200 patients with on cough of middle and advanced stage lung cancer admitted to the tumor outpatient,Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University from March 2021 to March 2024,and they were divided into the observation group and the control group according to the different treatment plans,100 cases in each group.The observation group was treated with ephedra soup plus flavor,and the control group was treated with Zhike Huatan Dingchuan pill.After 1 month of treatment,cough relief,changes in TCM evidence points,inflammatory factors(CRP,IL-6),eosinophil percentage(EOS%)before and after treatment,and adverse reactions were compared between the two groups.Results The total cough relief rate in the observation group was 95.00%,higher than 81.00%in the control group(χ^(2)=9.280,P=0.002).After 1 month of treatment,the symptom scores of cough and phlegm,wheezing,chest tightness and chest pain,lumbar and knee weakness,fatigue,nausea and vomiting,the levels of CRP,IL-6 and EOS%of the two groups were lower than those before treatment,and the observation group was lower than that of the control group(P<0.01).The difference in the total incidence of adverse reactions between the observation and control groups was not statistically significant(6.00%vs.11.00%,χ^(2)=1.607,P=0.205).Conclusion The clinical therapeutic effect of adding flavor to ephedra soup for treating cough in middle and advanced lung cancer is better,which can significantly improve patients′Chinese medicine symptoms,relieve the level of inflammatory reaction,and do not increase the adverse reactions,with higher safety.
