补肾益髓汤联合经颅磁刺激对脑梗死后认知障碍40例认知功能、血流动力学的影响

摘　　要：目的探究补肾益髓汤联合经颅磁刺激对脑梗死后认知障碍病人认知功能、血流动力学的影响。方法选择2021年1月至2022年1月秦皇岛市中医医院收治的脑梗死后认知障碍病人80例,按照随机数字表法分为两组,分别为颅磁组(n=40,采用经颅磁刺激疗法)和补肾汤组(n=40,在颅磁组基础上采用补肾益髓汤),比较两组病人治疗效果。结果补肾汤组总有效率(90.00%,36/40)高于颅磁组(70.00%,28/40)(P<0.05)。治疗后,补肾汤组和颅磁组病人MoCA、CDT、MMSE评分、生活自理能力评分、Vm较治疗前显著升高,且补肾汤组评分明显高于颅磁组(P<0.05)。PI、RI较治疗前显著降低,且补肾汤组明显低于颅磁组(P<0.05)。补肾汤组和颅磁组不良反应发生率比较差异无统计学意义(P>0.05)。结论补肾益髓汤联合经颅磁刺激对脑梗死后认知障碍病人治疗效果较好,可以改善病人的认知功能和生活自理能力,改善脑循环,增加脑部的供血和血液灌注,且药物使用更加安全。Objective To explore the influences of Bushen Yisui decoction combined with transcranial magnetic stimulation on cognitive function and hemodynamics in patients with cognitive impairment after cerebral infarction.Methods A total of 80 patients with cognitive impairment after cerebral infarction admitted to Qinhuangdao Traditional Chinese Medicine Hospital from January 2021 to January 2022 were divided into two groups according to random number table method,namely the transcranial magnetic group(n=40,using transcranial magnetic stimulation therapy)and the tonifying kidney group(n=40,using Busrenal Yisui decoction on the basis of the transcranial magnetic group),and the therapeutic effects of the two groups were compared.Results The total effective rate of the tonifying kidney group(90.00%,36/40)was higher than that of the transcranial magnetic group(70.00%,28/40)(P<0.05).After treatment,the scores of MoCA,CDT,MMSE,self-care ability and Vm in tonifying kidney group and transcranial magnetic group were significantly higher than those before treatment,and the scores in tonifying kidney group were significantly higher than those in transcranial magnetic group(P<0.05).PI and RI were significantly lower than those before treatment,and the tonifying kidney group was significantly lower than the transcranial magnetic group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group and the transcranial magnetic group(P>0.05).Conclusion Bushen Yisui decoction combined with transcranial magnetic stimulation has a good therapeutic effect on patients with cognitive impairment after cerebral infarction,which can improve cognitive function and self-care ability of patients,improves brain circulation,increase cerebral tissue blood perfusion of patients,and has drug safety.
