1242例农村癫痫患者的心理状况调查与分析

摘　　要：目的探究1242例陕西省农村地区癫痫患者的心理状况及其影响因素。方法对2012年2月至2020年9月入组陕西省农村癫痫防治管理项目的惊厥性癫痫患者开展横断面研究。通过问卷调查收集癫痫患者的社会人口学、发作症状学信息,并对患者的癫痫相关心理负担评分。采用机器学习领域中的k-means聚类算法,根据癫痫相关心理负担得分将患者分为低心理负担和高心理负担两组。采用多因素logistic回归分析癫痫相关心理负担的影响因素。结果共有1242例患者参与了本研究。1109例(89.3%)患者表现出不同程度的癫痫相关心理负担,癫痫复发是患者普遍最担心的问题,占比达到77.4%。高心理负担组患者454例(36.6%),本组患者最担心的问题是意外受伤,占比高达96.9%。多因素logistic回归分析表明,未婚/离异状态、治疗前发作频次≥24次/年、家庭年收入<36000元、近一年发作未控制是农村癫痫患者较高心理负担的危险因素(P<0.05)。结论近90%的患者表现出不同程度的癫痫相关心理负担。婚姻状态、家庭年收入及癫痫发作情况与农村患者癫痫相关心理负担显著相关。需针对农村患者加强癫痫知识宣教,提供社会支持和心理干预,以改善患者的生活质量。Objective To explore the psychological status and influencing factors of 1242 epilepsy patients in rural areas of Shaanxi Province.Methods A cross-sectional study was conducted on patients with convulsive epilepsy who were enrolled in the Shaanxi Province Rural Epilepsy Prevention and Management Project from February 2012 to September 2020.The sociodemographic data and seizure symptomatology information of epilepsy patients were collected through questionnaire survey,and the epilepsy-related psychological burden score was assessed.K-means clustering algorithm in the field of machine learning was used to divide the epilepsy patients into low psychological burden and high psychological burden groups based on the scores of epilepsy-related psychological burden.Multivariate logistic regression analysis was used to analyze the influencing factors of epilepsy-related psychological burden.Results A total of 1242 patients participated in this study,and 1109(89.3%)patients showed varying degrees of epilepsy-related psychological burden,with the most common concern being epilepsy recurrence,accounting for 77.4%.There were 454(36.6%)patients in the high psychological burden group,with the most concerning issue being accidental injury,accounting for 96.9%.Multivariate logistic regression analysis showed that unmarried/divorced status,seizure frequency≥24 times/year before treatment,annual family income<36000 yuan,and uncontrolled seizures in the past year were risk factors for higher psychological burden in epilepsy patients in rural areas(P<0.05).Conclusion Nearly 90%of the patients show varying degrees of epilepsy-related psychological burden.Marital status,annual family income,and the condition of seizures are significantly correlated with epilepsy-related psychological burden in rural patients.It is necessary to strengthen epilepsy knowledge education and provide social support and psychological intervention for rural patients to improve their quality of life.
