摘　　要：目的系统评价老年人慢性牙周炎的影响因素,为预防和减少老年人慢性牙周炎发生提供科学依据。方法在中国知网、中国生物医学数据库、维普期刊资源整合服务平台以及万方数据知识服务平台、PubMed、CINAHL、Web of Science、Embase、The Cochrane Library上进行检索,并辅助以人工检索,检索时限均从建库至2023年12月。由两位评价者独立筛选文献、提取资料和质量评价,采用Review Manager 5.3和Stata 17.0对符合纳入标准的文献进行Meta分析。结果共纳入12篇文献,总样本量为6611例,共研究22项危险因素。Meta分析结果显示,女性、初中及以上学历、就医行为是老年人群慢性牙周炎的保护因素(OR<1.0,P<0.05);吸烟、戒烟、糖尿病、合并系统性疾病、有不良修复体、合并根分叉病变、口腔健康认知情况差是老年人群慢性牙周炎的危险因素(OR>1.0,P<0.05)。结论影响老年人群慢性牙周炎的因素较多,医务人员可结合临床选择可控的因素,进行早期干预,降低老年人群发生慢性牙周炎的风险,提高老年人群生活质量。Objective To systematically evaluate the influencing factors of chronic periodontitis in the elderly,and to provide a scientific basis for preventing and reducing the occurrence of chronic periodontitis in the elderly.Methods Searches were conducted on CNKI,CBM,VIP,and Wanfang,PubMed,CINAHL,Web of Science,Embase,and The Cochrane Library,supplemented by manual searches,and the timeframe for the searches was from the establishment of the library to December 2023 in all cases.Literature was independently screened,extracted and quality assessed by 2 evaluators,and Meta-analysis was performed using Review Manager 5.3 and Stata 17.0 on literature that met the inclusion criteria.Results A total of 12papers were included,with a total sample size of 6611 cases,and a total of 22 risk factors were studied.Meta-analysis showed that female,junior high school education and above,and healthcare-seeking behaviors were the protective factors for chronic periodontitis in the elderly population(OR<1.0,P<0.05);smoking,smoking cessation,diabetes mellitus,co-morbid systemic diseases,the presence of defective restorations,co-morbid root-fracture pathology,and a poor oral health awareness were the risk factors for chronic periodontitis in the elderly population(OR>1.0,P<0.05).Conclusion There are many factors affecting chronic periodontitis in the elderly population,and medical personnel can choose the controllable factors combined with the clinic for early intervention to reduce the risk of chronic periodontitis and improve the quality of life in the elderly population.
