摘 要:目的探讨因甲状腺结节行手术治疗的成年患者体重指数(BMI)与多灶性甲状腺乳头状癌(MPTC)发生风险的相关性。方法选取2016年1月至2020年12月在解放军总医院第一医学中心因甲状腺结节行全切手术治疗并确诊甲状腺乳头状癌的患者4651例,分为单灶性甲状腺乳头状癌(UPTC)组(2180例)及MPTC组(2471例),比较两组间相关临床及病理特征资料。采用logistic回归分析BMI与MPTC发生风险的相关性,并基于性别、年龄分层进行亚组分析。结果与UPTC组相比,MPTC组BMI水平更高且超重及肥胖患者占比更高(P均<0.001)。调整混杂因素后,BMI水平升高与MPTC风险增加有关(OR=1.047,95%CI:1.029~1.066,P<0.001)。相较于正常体重患者,超重(OR=1.179,95%CI:1.031~1.348,P=0.016)及肥胖(OR=1.555,95%CI:1.314~1.840,P<0.001)患者MPTC发生风险增加。按性别、年龄分层进行亚组分析后发现,无论男性或女性,仅在年龄<55岁患者中BMI水平增加与MPTC患病风险升高有关(OR=1.057,95%CI:1.037~1.078,P<0.001)。结论高BMI水平与MPTC发生风险增加相关,超重和肥胖是<55岁患者发生MPTC的独立危险因素。ObjectiveTo investigate the association between body mass index(BMI)and the risk of multifocal papillary thyroid carcinoma(MPTC)in adult patients who underwent thyroidectomy for thyroid nodules.MethodsA total of 4651 patients diagnosed with papillary thyroid carcinoma after undergoing thyroidectomy for thyroid nodules from January 2016 to December 2020 were selected.The patients were divided into two groups:unifocal papillary thyroid carcinoma(UPTC)and MPTC,and the relevant clinical and pathological characteristics were compared between the two groups.Logistic regression analysis was used to analyze the association between BMI and MPTC risk,and subgroup analysis was conducted based on gender and age stratification.ResultsCompared with the UPTC group,the MPTC group had a higher BMI level and a higher proportion of overweight and obese patients(P<0.001).After adjusting for confounding factors,a higher BMI level was associated with an increased risk of MPTC(OR=1.047,95%CI:1.029-1.066,P<0.001).Compared with normal weight patients,overweight(OR=1.179,95%CI:1.031-1.348,P=0.016)and obesity(OR=1.555,95%CI:1.314-1.840,P<0.001)patients had an increased risk of MPTC.After conducting subgroup analyses stratified by gender and age,it was found that,regardless of gender,an increase in BMI level was associated with an elevated risk of MPTC only in patients aged<55 years(OR=1.057,95%CI:1.037-1.078,P<0.001).ConclusionHigh BMI level is associated with an increased risk of MPTC.Overweight and obesity are independent risk factors for MPTC in patients younger than 55 years old undergoing surgical treatment for thyroid nodules.
