摘 要:目的探讨实时荧光定量PCR(qRT-PCR)检测急性粒细胞白血病部分分化型(AML-M2)患者骨髓中的WT1基因表达水平及临床意义。方法采用qRT-PCR检测67例AML-M2患者及30名健康者骨髓样本中的WT1基因表达水平。根据AML1-ETO融合基因状态将AML-M2患者分为AML1-ETO阳性组和阴性组,比较两组的WT1基因表达水平及AML1-ETO与WT1基因检测的诊断价值。将AML1-ETO阴性患者按WT1基因表达水平分为低表达组和高表达组,分析其与原始细胞比例、完全缓解(CR)、无事件生存时间(EFS)及总生存时间(OS)的关系。结果AML-M2患者的WT1基因中位表达水平高于健康者(P=0.000)。AML1-ETO阳性组中AML1-ETO基因检测和WT1基因检测的曲线下面积(AUC)无显著差异(P>0.05)。WT1基因低表达组和WT1基因高表达组的原始细胞比例有显著差异(P=0.000);Spearman相关性分析结果显示,WT1基因表达水平与原始细胞比例呈正相关(P=0.000);9例AML1-ETO阴性的AML-M2患者的WT1基因表达水平与原始细胞比例呈正相关(P<0.05)。WT1基因低表达组的OS短于WT1基因高表达组(P=0.031)。结论骨髓中WT1基因表达水平对AML-M2患者的诊断、治疗及预后评估具有重要临床意义。Objective To investigate the expression level of WT1 gene in bone marrow of patients with acute myeloblastic leukemia-M2(AML-M2)detected by real-time fluorescence quantitative PCR(qRT-PCR)and its clinical significance.Methods The expression level of WT1 gene in bone marrow samples of 67 AML-M2 patients and 30 healthy controls was detected by qRT-PCR.According to the status of AML1-ETO fusion gene,the AML-M2 patients were divided into AML1-ETO positive group and negative group;the expression level of WT1 gene and the diagnostic value of AML1-ETO and WT1 gene detection were compared between the two groups.The AML1-ETO negative patients were divided into low expression group and high expression group according to the expression level of WT1 gene,and their relationship with the proportion of initial cells,complete response(CR),event-free survival(EFS)and overall survival(OS)were analyzed.Results The median expression level of WT1 gene in AML-M2 patients was higher than that in healthy subjects(P=0.000).There was no significant difference in the area under curve(AUC)between AML1-ETO gene detection and WT1 gene detection in the AML1-ETO positive group(P>0.05).There was a significant difference in the proportion of initial cells between the WT1 gene low expression group and the WT1 gene high expression group(P=0.000);Spearman correlation analysis results showed that the expression level of WT1 gene was positively correlated with the proportion of initial cells(P=0.000);the expression level of WT1 gene in 9 AML-M2 patients with negative AML1-ETO was positively correlated with the proportion of initial cells(P<0.05).The OS of the WT1 gene low expression group was shorter than that of the WT1 gene high expression group(P=0.031).Conclusion The expression level of WT1 gene in bone marrow has important clinical significance for the diagnosis,treatment and prognosis evaluation of AML-M2 patients.
