阿加曲班联合抗血小板药物在进展性脑梗死中的应用效果

摘　　要：目的探究阿加曲班联合抗血小板药物在进展性脑梗死(PIS)中的应用效果。方法选取2018年1月1日至2023年4月30日湖南省娄底市中心医院神经内科住院治疗的80例PIS患者作为研究对象,根据治疗方式将其分为对照组(50例)与治疗组(30例)。患者入院后给予脑梗死常规治疗,病情进展后,对照组在常规治疗基础上予以抗血小板药物治疗,治疗组则在常规治疗基础上予以阿加曲班联合抗血小板药物治疗。比较两组的神经功能缺损情况、预后情况、病情缓解时间及治疗效果。结果治疗后,对照组的美国国立卫生研究院卒中量表(NIHSS)、改良Rankin量表(mRS)评分均高于治疗前(P<0.05);治疗后,治疗组的mRS评分低于治疗前(P<0.05);治疗后,治疗组的NIHSS、mRS评分均低于对照组(P<0.05);治疗前、后,治疗组的NIHSS评分无显著差异(P>0.05)。治疗组的病情缓解时间短于对照组,治疗总有效率高于对照组(P<0.05)。结论阿加曲班联合抗血小板药物治疗PIS临床效果确切,可有效缓解病情进展,改善患者的神经功能缺损状况,降低残疾率,值得在临床实践中进一步推广及应用。Objective To explore the application effect of argatroban combined with antiplatelet drugs in progressive ischemic stroke(PIS).Methods A total of 80 patients with PIS who were hospitalized in the neurology department of Loudi Central Hospital of Hunan Province from January 1,2018 to April 30,2023 were selected as the research objects.According to the treatment methods,the patients were divided into control group(50 cases)and treatment group(30 cases).After admission,the patients were given routine treatment of cerebral infarction.After the disease progressed,the control group was treated with antiplatelet drugs on the basis of routine treatment,and the treatment group was treated with argatroban combined with antiplatelet drugs on the basis of routine treatment.The neurological deficit,prognosis,remission time and therapeutic effect of the two groups were compared.Results After treatment,the scores of National Institutes of Health Stroke Scale(NIHSS)and modified Rankin Scale(mRS)in the control group were higher than those before treatment(P<0.05);after treatment,the mRS score of the treatment group was lower than that before treatment(P<0.05);after treatment,the NIHSS and mRS scores of the treatment group were lower than those of the control group(P<0.05);before and after treatment,there was no significant difference in NIHSS score in the treatment group(P>0.05).The remission time of the treatment group was shorter than that of the control group,and the total effective rate of treatment was higher than that of the control group(P<0.05).Conclusion The argatroban combined with antiplatelet drugs in the treatment of PIS has definite clinical effect,which can effectively alleviate the progression of the disease,improve the neurological deficits of patients,and reduce the incidence of disability.It is worthy of further promotion and application in clinical practice.
