摘　　要：本文报道了一例梗阻性肥厚型心肌病的老年男性患者,行皮腔内室间隔心肌消融术,术后心功能状况良好,无心力衰竭的典型临床表现。术后14年超声心动图发现心尖部室壁瘤形成,经心脏核磁共振和冠状动脉造影进一步明确诊断,其间出现急性脑梗死,可能与心尖部室壁瘤和心房纤颤有密切关系,给予抗凝及改善心肌重构药物治疗,目前临床预后良好。This report describes an elderly male patient with obstructive hypertrophic cardiomyopathy who underwent percutaneous intracavitary septal myocardial ablation.After operation,the patient exhibited good cardiac function without typical clinical manifestations of heart failure.Fourteen years later,echocardiography revealed the formation of an apical ventricular aneurysm,which was further confirmed by cardiac magnetic resonance imaging(MRI)and coronary angiography.During this period,the patient experienced an acute cerebral infarction,which may be related to the apical ventricular aneurysm and atrial fibrillation.Treatment with anticoagulation and medications to improve myocardial remodeling was initiated,resulting in a favorable clinical prognosis.
