早期食管癌患者内镜下黏膜剥离术后生活质量的影响因素分析

摘　　要：目的分析影响早期食管癌患者内镜下黏膜剥离术(ESD)后生活质量的因素。方法采用便利抽样法,选取2021年8月—2023年7月沧州市中心医院的162例早期食管癌ESD术后6个月按时复诊的患者为研究对象。采用一般资料调查问卷、食管癌生活质量量表(QLICP-ES)对患者进行调查。采用单因素分析和多重线性回归分析对早期食管癌ESD术后患者生活质量的影响因素进行分析。结果单因素分析结果显示,不同性别、年龄、婚姻状况、年收入、医疗付费方式、伴随疾病、浸润程度、脉管侵犯、手术并发症、切缘阳性的患者QLICP-ES评分比较差异均有统计学意义(P<0.05)。多重线性回归分析结果显示,年龄、婚姻状况、伴随疾病、脉管侵犯、手术并发症、切缘阳性是早期食管癌ESD术后患者QLICP-ES评分的影响因素(P<0.05)。结论早期食管癌患者ESD术后6个月生活质量处于中高水平,高龄、未婚/离婚/丧偶、存在伴随疾病、伴脉管侵犯、伴手术并发症及切缘阳性早期食管癌ESD术后患者QLICP-ES量表评分较低,建议临床对上述因素予以重点关注,必要时采取针对性护理措施。ObjectiveTo analyze the factors affecting quality of life in patients with early esophageal cancer after endoscopic submucosal dissection(ESD).MethodsConvenience sampling was used to select 162 early esophageal cancer patients with ESD in Cangzhou Central Hospital from August 2021 to July 2023 who had an on-time follow-up six months as study subjects.General Information Questionnaire and Quality of Life Instruments for Cancer Patients-Esophageal Cancer(QLICP-ES)were used to investigate the patients.Factors affecting the quality of life of patients after ESD for early esophageal cancer were analyzed using univariate analysis and multiple linear regression analysis.ResultsUnivariate analysis showed statistically significant differences in QLICP-ES scores among patients with different gender,age,marital status,annual income,medical payment method,concomitant diseases,degree of infiltration,vascular invasion,surgical complications,and positive margins(P<0.05).Multiple linear regression analysis showed that age,marital status,concomitant diseases,vascular invasion,surgical complications,and positive margins were the factors influencing the QLICP-ES scores of patients with ESD for early esophageal cancer,and the differences were statistically significant(P<0.05).ConclusionsQuality of life at six months after ESD in patients with early esophageal cancer is at an intermediate to high level.Patients of advanced age,unmarried/divorced/widowed,concomitant diseases,vascular invasion,surgical complications and positive margins have low QLICP-ES scores after ESD for early esophageal cancer.It is recommended that clinical attention be focused on these factors and that targeted nursing measures be taken when necessary.
