摘 要:目的:基于定量CT(QCT)研究精神分裂症患者腰椎骨密度(BMD)与正常人群的异同及临床相关因素。方法:回顾性选取119名40岁以上住院治疗的精神分裂症患者作为患者组。同时收集健康体检人群,并按照性别与年龄进行1:1病例对照匹配。所有研究对象均通过QCT来测量腰椎BMD,基于性别分层比较患者组和对照组之间BMD的差别,采用二元Logistic回归模型探索可能影响腰椎BMD的其他因素。结果:相对对照组,患者组平均腰椎BMD较低,骨质疏松率高。年龄对BMD具有显著的负向影响(OR=0.872,95%CI:0.806~0.928,P<0.001)。经过年龄校准后,性别、体质量指数(BMI)、高血压与BMD是否正常无显著相关性(P>0.05)。结论:精神分裂症住院患者骨质疏松的患病率较对照组高,精神分裂症患者的BMD明显减低。腰椎BMD与患精神分裂症以及年龄显著相关。Objective:The study aims to explore the differences in lumbar bone mineral density(BMD)between patients with schizophrenia and healthy controls,as well as the associated clinical factors,using quantitative CT(QCT).Methods:A retrospective study included 119 hospitalized Patients with Schizophrenia aged≥40 years as the patient group.Healthy individuals undergoing physical examinations were recruited as controls and matched 1:1 by gender and age.Lumbar BMD was measured by QCT in all subjects.Gender-stratified comparisons of BMD were performed between groups.Binary logistic regression was used to analyze potential factors influencing BMD.Results:Compared with the control group,the patient group demonstrated a lower mean lumbar BMD and a higher osteoporosis rate.Binary logistic regression analysis revealed a negative correlation between age and BMD(OR=0.872,95%CI:0.806~0.928,P<0.001).After adjusting for age,gender,body mass index,and hypertension were not significantly associated with BMD(P>0.05).Conclusion:The findings of this study demonstrate that hospitalized patients with schizophrenia exhibit a higher prevalence of osteoporosis and significantly reduced BMD compared to healthy controls.This results indicate that lumbar BMD is significantly associated with schizophrenia and age.
