摘 要:目的探究超声乳腺影像报告和数据系统(BI-RADS)超声量化评分、弹性应变率比值(SR)联合血清抗癌原153(CA153)在乳腺结节良恶性中的价值。方法以病理组织检查结果为“金标准”选取2022年1月至2024年1月佳木斯市中心医院东院区物理诊断科接收的乳腺结节患者83例(恶性结节52例,良性结节31例)进行回顾性分析,收集所有研究对象BI-RADS量化评分、SR及血清CA153,绘制列线图进行诊断预测模型构建,并通过绘制受试者特征曲线(ROC)进行内部验证评估列线图模型预测能力。结果以病理检测结果为“金标准”评估BI-RADS量化评分、SR诊断乳腺结节的准确性为84.34%、86.75%,敏感度为91.49%、91.84%,特异度为75.00%、79.41%;恶性乳腺结节患者血清CA153指标高于良性乳腺结节患者(t=17.651,P<0.05);建立列线图预测模型,ROC曲线中AUC值为0.999(95%CI:0.998~1),由此表示该临床预测模型具有较好的风险预测度。结论BI-RADS量化评分、SR及血清CA153在乳腺结节良恶性鉴别中具有较高临床价值,基于上述指标构建预测模型能够筛查高危群体,为临床实践提供理论指导。Objective To investigate the value of ultrasonic quantization score and elastic strain rate ratio(SR)combined with serum anticancer Pro153(CA153)in benign and malignant breast nodules by Ultrasonic Breast Imaging Report and Data System(BI-RADS).Methods A total of 83 patients with breast nodules(52 malignant and 31 benign),selected based on pathological tissue examination results as the"gold standard,"were included in this retrospective analysis.These patients were treated at the Physical Diagnosis Department of East Hospital of Jiamusi Central Hospital,from January 2022 to January 2024.The BI-RADS quantitative score,SR,and serum CA153 of all subjects were collected.The diagnostic prediction model was constructed by drawing a nomogram,and the predictive ability of the nomogram model was evaluated by drawing receiver characteristic curve(ROC curve)for internal verification.Results The accuracy of BI-RADS quantitative score and SR diagnosis of breast nodules,evaluated based on pathological examination results as the"gold standard,were 84.34%and 86.75%,the sensitivity was 91.49%and 91.84%,and the specificity was 75.00%and 79.41%.The serum CA153 index of malignant breast nodules was higher than that of benign breast nodules(t=17.651,P<0.05).A nomogram prediction model was established,and the AUC value in the ROC curve was 0.999(95%CI:0.998-1),indicating that the clinical prediction model had a good risk prediction degree.Conclusion BI-RADS quantitative score,SR,and serum CA153 have high clinical value in the identification of benign and malignant breast nodules.The prediction model based on the above indicators can screen high-risk groups and provide theoretical guidance for clinical practice.
