摘　　要：目的探讨龈沟液缝隙连接蛋白43(Cx43)、组织金属蛋白酶抑制剂-1(TIMP-1)水平与不可复性牙髓炎(IP)患者根管治疗效果的关系。方法选取2021年1月至2022年6月在张家港市第一人民医院行根管治疗的IP患者153例(IP组)和同期健康体检志愿者80名(对照组),根据根管治疗效果将IP患者分为不良组(31例)和良好组(122例),采用酶联免疫吸附法检测龈沟液Cx43、TIMP-1水平。以IP患者根管治疗效果为因变量,多因素条件Logistic回归确定其影响因素,绘制受试者工作特征曲线(ROC)分析龈沟液Cx43、TIMP-1水平对其的评估能效。结果IP组龈沟液Cx43、TIMP-1水平高于对照组(P<0.05)。Cx43高、TIMP-1高为根管治疗后预后不良的独立危险因素(P<0.05)。龈沟液Cx43、TIMP-1水平联合评估IP患者根管治疗后预后不良的曲线下面积(AUC)为0.880,二者单独评估的0.789、0.796(P<0.05)。结论IP患者龈沟液Cx43、TIMP-1水平升高,与根管治疗后预后不良相关,二者联合对其预测能效较高。Objective To explore the relationship between gingival crevicular fluid connexin 43(Cx43)and tissue inhibitor of metalloproteinases-1(TIMP-1)levels and the outcome of root canal treatment in patients with irreversible pulpitis(IP).Methods A total of 153 IP patients who underwent root canal treatment between January 2021 and June 2022(IP group)and 80 healthy volunteers(control group)were selected.IP patients were further divided into the unfavorable outcome group(31 cases)and the favorable outcome group(122 cases)based on treatment results.Enzyme-linked immunosorbent assay(ELISA)was used to measure Cx43 and TIMP-1 levels in gingival crevicular fluid.Logistic regression analysis was performed to identify factors influencing treatment outcomes,and receiver operating characteristic(ROC)analysis was conducted to evaluate the predictive performance of Cx43 and TIMP-1 levels.Results Cx43 and TIMP-1 levels were significantly higher in the IP group compared to the control group(P<0.05).High levels of Cx43 and TIMP-1 were independent risk factors for poor prognosis after root canal treatment(P<0.05).The area under the ROC curve for combined Cx43 and TIMP-1 levels in predicting poor prognosis was 0.880,higher than for Cx43 or TIMP-1 alone(0.789 and 0.796,respectively;P<0.05).Conclusion Elevated levels of Cx43 and TIMP-1 in gingival crevicular fluid are associated with poor outcomes after root canal treatment in IP patients,with high predictive performance when combined.
