血清miR-15b、TLR7水平与阿尔茨海默病患者神经炎症及认知功能的关系

摘  要:目的探讨血清微小RNA-15b(miR-15b)、Toll样受体7(TLR7)水平与阿尔茨海默病(AD)患者神经炎症及认知功能的关系。方法选取2022年6月至2024年6月河北省第三荣军优抚医院收治的AD患者78例(AD组)和健康体检者32例(对照组)为研究对象,根据认知功能将AD患者分为正常组(n=25)、轻度认知功能障碍(MCI)组(n=35)和痴呆组(n=18)。检测血清miR-15b、TLR7和神经炎症因子水平,通过Pearson相关分析AD患者血清miR-15b、TLR7与神经炎症因子的相关性;采用受试者工作特征曲线分析血清miR-15b、TLR7水平对AD患者认知功能的评估能效。结果AD组血清miR-15b水平和蒙特利尔认知评估量表(MoCA)评分均低于对照组,TLR7、白细胞介素1β(IL-1β)、IL-6、肿瘤坏死因子α(TNF-α)水平均高于对照组(P均<0.05)。AD组血清miR-15b水平与TLR7、IL-1β、IL-6、TNF-α水平均呈负相关,血清TLR7与IL-1β、IL-6、TNF-α水平均呈正相关(P均<0.05)。正常组、MCI组、痴呆组血清miR-15b水平依次降低,血清TLR7、IL-1β、IL-6、TNF-α水平依次升高(P均<0.05)。血清miR-15b、TLR7联合评估AD患者MCI、痴呆的曲线下面积(AUC)分别为0.869、0.875,大于血清miR-15b(0.793、0.789)、TLR7(0.808、0.792)单独评估的AUC(P均<0.05)。结论AD患者血清miR-15b水平降低、TLR7水平升高与神经炎症加重和认知功能降低相关,可能成为AD患者认知功能评估的重要指标。Objective To explore the relationship between serum microRNA-15b(miR-15b),Toll-like receptor 7(TLR7)levels and neuroinflammation as well as cognitive function in patients with Alzheimer's disease(AD).Methods A total of 78 AD patients(AD group)and 32 healthy controls were selected from June 2022 to June 2024 in the Third Rongjun Special Care Hospital of Hebei Province.AD patients were divided into normal group(n=25),mild cognitive impairment(MCI)group(n=35),and dementia group(n=18)according to cognitive function.Serum miR-15b,TLR7,and neuroinflammatory factors were detected.Pearson correlation analysis was used to analyze the correlation between serum miR-15b,TLR7,and neuroinflammatory factors in AD patients.Receiver operating characteristic curves were used to analyze the evaluation efficiency of serum miR-15b and TLR7 levels for cognitive function in AD patients.Results In the AD group,the serum miR-15b level and Montreal Cognitive Assessment(MoCA)score were lower than those in the control group,while the levels of TLR7,interleukin-1β(IL-1β),IL-6,and tumor necrosis factor-α(TNF-α)were higher(all P<0.05).In the AD group,the serum miR-15b level was negatively correlated with the levels of TLR7,IL-1β,IL-6,and TNF-α,and the serum TLR7 level was positively correlated with the levels of IL-1β,IL-6,and TNF-α(all P<0.05).In the normal,MCI,and dementia groups,the serum miR-15b level decreased in turn,and the levels of serum TLR7,IL-1β,IL-6,and TNF-αincreased in turn(all P<0.05).The areas under the curve(AUCs)of the combined evaluation of serum miR-15b and TLR7 for MCI and dementia in AD patients were 0.869 and 0.875,respectively,which were larger than those of serum miR-15b(0.793,0.789)and TLR7(0.808,0.792)alone(all P<0.05).Conclusion The decrease in serum miR-15b level and the increase in TLR7 level in AD patients are related to the aggravation of neuroinflammation and the decline of cognitive function,and may become important indicators for the evaluation of cognitive function in AD patients.
