摘 要:目的 探究肥胖儿童血清25-羟维生素D[25(OH)D]水平对其糖脂代谢的影响。方法 选取2023年6月至2024年6月在江西省儿童医院诊断为儿童单纯性肥胖并糖脂代谢异常的7~14岁患儿101例作为研究对象,应用随机数字表法将其分为观察组(n=51)和对照组(n=50)。观察组给予维生素D3补充治疗,2组均接受营养师减重期饮食指导联合运动管理。比较2组治疗前后的体重指数(BMI)、血清25(OH)D水平和糖脂代谢指标,并分析治疗前血清25(OH)D水平与BMI和糖脂代谢指标的相关性。结果 治疗前,患儿血清25(OH)D水平与空腹胰岛素水平(FINS)、胰岛素抵抗指数(HOMA-IR)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)呈负相关,与高密度脂蛋白胆固醇(HDL-C)呈正相关(均P <0.05)。治疗后,观察组BMI、FINS、HOMA-IR、TG、TC和LDL-C水平显著低于对照组,而血清25(OH)D和HDL-C水平显著高于对照组(均P <0.05)。结论 肥胖儿童血清25(OH)D水平与其糖脂代谢指标有相关性,单纯性肥胖儿童在饮食联合运动干预的基础上口服维生素D3治疗,有利于其糖脂代谢的改善。Objective To investigate the impact of serum 25-hydroxyvitamin D(25(OH)D)levels on glucose and lipid metabolism in children with obesity.Methods A cohort of 101 children aged 7–14 years diagnosed with simple obesity and glucose/lipid metabolic abnormalities at Jiangxi Provincial Children’s Hospital(June 2023–June 2024)was randomized via a random number table into an observation group(n=51,receiving vitamin D3 supplementation)and a control group(n=50).Both groups underwent dietitian-guided weight-loss dietary interventions combined with exercise management.Body mass index(BMI),serum 25(OH)D levels,and glucose/lipid metabolic parameters were compared pre-and post-intervention,with correlations analyzed between baseline 25(OH)D levels and metabolic indices.Results Pretreatment serum 25(OH)D levels inversely correlated with fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR),triglycerides(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C),while positively correlating with high-density lipoprotein cholesterol(HDL-C)(all P<0.05).Post-treatment,the observation group exhibited significantly lower BMI,FINS,HOMA-IR,TG,TC,and LDL-C levels,alongside higher 25(OH)D and HDL-C levels compared to controls(all P<0.05).Conclusion Serum 25(OH)D levels demonstrate significant correlations with glucose/lipid metabolic profiles in obese children.Adjunctive vitamin D3 supplementation to standard lifestyle interventions may facilitate improvement of metabolic dysregulation in pediatric simple obesity.
