加减虎潜丸联合羟考酮缓释片治疗恶性肿瘤骨转移疼痛(肝肾亏虚证)的临床观察

摘　　要：目的观察加减虎潜丸联合羟考酮缓释片治疗恶性肿瘤骨转移疼痛(肝肾亏虚证)的临床疗效。方法按照随机数字表法,将86例肝肾亏虚型恶性肿瘤骨转移疼痛的患者分为观察组和对照组各43例,观察组接受加减虎潜丸联合羟考酮缓释片治疗,对照组接受羟考酮缓释片治疗,疗程14 d,比较两组疼痛评分、疼痛缓解率、爆发性疼痛次数、羟考酮使用量、生存质量评分、中医证候积分及不良反应。结果两组疼痛治疗总有效率比较无明显差异(P>0.05);观察组显愈率高于对照组(P<0.05);观察组治疗后疼痛NRS评分、羟考酮日用量及爆发痛次数均低于对照组,生存质量评分高于对照组(P<0.05);两组主证积分比较无明显差异(P>0.05);观察组次证积分和总积分均低于对照组(P<0.05);两组治疗期间均未出现明显不良反应。结论加减虎潜丸联合羟考酮缓释片治疗肝肾亏虚型恶性肿瘤骨转移疼痛效果显著,可有效减轻患者疼痛,减少阿片类药用量及爆发痛出现次数,使患者在晚期止痛治疗中获得更好的生存体验。Objectives To observe the clinical efficacy of Modified Huqian Pills combined with Oxycodone Sustainedrelease Tablets in the treatment of pain due to malignant tumor bone metastases(liver and kidney deficiency syndrome).Methods According to the random number table method,86 patients with pain due to malignant tumor bone metastases of liver and kidney deficiency type were divided into observation group and control group,with 43 cases in each group.The observation group received Modified Huqian Pills combined with Oxycodone Sustained-release Tablets,while the control group received Oxycodone Sustained-release Tablets for 14 days.The pain scores,pain relief rates,number of breakthrough pain episodes,oxycodone dosage,quality of life scores,Traditional Chinese Medicine(TCM)syndrome scores,and adverse reactions were compared between the two groups.Results There was no significant difference in the total effective rate of pain treatment between the two groups(P>0.05).The markedly improvement and cure rate was significantly higher in the observation group than that in the control group(P<0.05).After treatment,the NRS pain score,daily oxycodone dosage,and number of breakthroughpain episodes were significantly lower in the observation group than those in the control group,while the quality of life score was significantly higher(P<0.05).There was no significant difference in the main syndrome scores between the two groups(P>0.05).The secondary syndromescores and total scoreswere significantlylower in the observation group than those in the control group(P<0.05).No significant adverse reaction was observed during treatment in either group.Conclusions ModifiedHuqian Pills combined with Oxycodone Sustained-release Tablets is effective in treating pain due to malignant tumor bone metastases of liver and kidney deficiency type,which can effectively alleviate patients’pain,reduce the dosage of opioids and the number of breakthrough pain episodes,which provide patients with better quality of life experience during late-stage pai
